In consideration of my child’s participation in athletic and recreational activities offered by Ummah Athletics Association (UAA), I, the undersigned parent or legal guardian, hereby agree to the following: 1. Assumption of Risk acknowledge that participation in any sports and fitness training involves inherent risks, including but not limited to physical injury, illness, falls, contact with other participants, and environmental hazards. I voluntarily assume all such risks on behalf of my child. 2. Medical Treatment Authorization - In the event of a medical emergency, I authorize UAA staff, coaches, or volunteers to obtain emergency medical treatment for my child and I agree to be financially responsible for any costs incurred as a result. 3. Waiver and Release - I hereby waive, release, and discharge Ummah Athletics Association, its board members, coaches, volunteers, affiliates, and sponsors from any and all liability, claims, demands, and causes of action arising out of or related to my child’s participation, including claims related to exposure of illness, negligence, injury, or death. 4. Code of Conduct - I understand that all participants are expected to follow UAA’s conduct guidelines and that failure to do so may result in removal from activities without refund.
