  • This Agreement is made between STUDIO58 Performance LLC (“Coach/Trainer”) and the undersigned (“Client”) and sets forth the understanding for coaching or training services.

     

    1. Session Terms

    • Session length, frequency, location, and schedule will be discussed and agreed upon between both parties before services begin.

    • A written or verbal schedule confirmation will serve as the basis for the service plan.

    2. Payment Terms

    • The rate, payment frequency, and method of payment will be discussed and agreed upon at the initial session.

    • Payment must be made according to the agreed schedule in order to continue services.

    3. Cancellations & Rescheduling

    • The Client agrees to provide at least 24 hours’ notice to cancel or reschedule any session.

    • Late cancellations may result in a forfeited session at the Coach/Trainer’s discretion.

    • The Coach/Trainer will also provide reasonable notice if rescheduling is required.

    4. Communication

    Both parties agree to maintain respectful, clear, and timely communication regarding scheduling, progress, and service expectations.

    5. Confidentiality

    Any personal or sensitive information shared will be kept confidential and used solely for the purpose of delivering the service, except where disclosure is required by law.

    6. Term & Termination

    This Agreement remains in effect until either party gives notice to end services. Any outstanding sessions or balances will be addressed at termination.

    7. Acknowledgment

    By signing below, the Client agrees to the general service terms outlined above and acknowledges that a separate liability waiver must also be signed prior to the start of services.
