Guardian Home Application
Thank you for reaching out to learn more about becoming a guardian home for one of our precious goldendoodles. We look forward to getting to know you and will reach out to schedule a phone call after we receive this application. First and foremost, our parents are beloved family pets. We created a Guardian-Home program to grow, while providing our breeding dogs with a loving home environment.A guardian home is a unique opportunity where a local family receives a well-socialized, temperament-tested, health-tested goldendoodle that is a current or future Teddy Puppies parent who is up-to-date on all vaccines for free, in exchange for Teddy Puppies to retain breeding rights as long as his/her health is optimal and we're able to communicate & work out the logistics effectively together. This dog becomes your family pet, and when they retire (typically before females turn 5 years old and around age 7 for males) from breeding, then full ownership is transferred to you. Males may not be used for stud services outside of my program without my consent and no compensation will be given to the guardian family. What is your living situation? Do you own your own home?

If you go out of town, what is your pet-sitting plan? Because this dog will not be fixed, they are unable to be boarded at facilities and in-home pet sitting is required, unless approval has been given. We need to know where this dog is at all times when entrusted to your care.

If you are approved, what veterinarian do you plan on using? Do you commit to maintain active pet insurance for this dog? In the unfortunate event that your dog needs emergency medical care, are you prepared to pay for that?

If you are approved, will you keep an active AirTag (or off-brand) or Fi Nano collar that allows you to GPS track them in real-time? Being able to find your dog quickly saves lives. Your dog will come microchipped, however that isn't helpful unless they get picked up by a rescue.

How do you plan to pay for pet-related emergency expenses? We give 1 month of free pet insurance with our guardian dogs, but you will be responsible for non-breeding pet-related health expenses.

Do you commit to contact Teddy Puppies on day 1 of bleeding for females (signifying the start of a heat cycle) and coordinate her to be dropped off at our house within a week? For females, we will provide prenatal vitamins when she goes back to live with you and she'll need to be dropped back off at our home 1 week before her due date. Once puppies are fully weaned (around 7 weeks) she will go back to live with you.

Guardian Home Agreement
This contract is between Teddy Puppies, LLC. and the Guardian Family.•Ownership and rights remain with the owner/breeder and all puppies born from this dog are the sole property of the owner/breeder. The Guardian family has no ownership interested of possession rights to any of the puppies.• This Teddy Puppies parent dog is entrusted to the Guardian Family, free-of-charge, as a family pet under this contract, with the condition that Teddy Puppies retains the breeding rights and ability to breed this dog. Female dogs are not permitted to interact with intact male dogs at any time.• The Guardian home authorizes Teddy Puppies to use any and all still photography and/or video recordings provided by the Guardian home family.• In the event a dispute shall arise between the parties to this contract, the parties agree to participate in at least four hours of mediation in accordance with the mediation procedures of United States Arbitration & Mediation. The parties agree to share equally in the costs of the mediation. The mediation shall be administered by the offices of United States Arbitration in the state of Georgia, county of Cherokee.Teddy Puppies Representations:The following conditions apply to Guardian Home co-owners:1. Teddy Puppies guarantees this dog is in good health, is up-to-date on immunizations appropriate for the puppy’s age, and has had appropriate worming at the time the Guardian Home assumes care.2. Teddy Puppies commits to pay for all reproductive-related expenses, including prenatal & postnatal vitamins, and the final spay/neuter procedure, upon which full ownership to the Guardian family will be transferred.3. Teddy Puppies agrees to transfer full ownership of this dog to the Guardian family when she turns 5 years old or retires from our program, based on a licensed veterinarian's recommendation.4. Teddy Puppies has the right to re-home this dog if the Guardian family is suspected of animal abuse, neglect, or hindering the breeding process in any way (including moving more than 1 hour away from zip code 30115).5. The owner/breeder is not responsible for any damage done by this dog to harm people, other dogs, or to property. It is the guardian family's responsibility to crate when not supervising this dog and provide age-appropriate training/discipline when necessary. The guardian family agrees to indemnify and hold the owner/breeder harmless from and against all claims, liabilities, losses, costs, damages, and expenses by reason of any damaged property, or injury allegedly caused by the dog while in the possession of the guardian family.6. No warranties or guarantees, expressed or implied, are made under this contract except as stated.Guardian Home Representations:1. The Guardian Family commits to feed the dog premium quality food, keep the dog at a healthy weight, provide annual veterinary appointments for the dog and keep all immunizations current according to their state guidelines of the AVMA, and properly administer heartworm, deworming (as needed), and flea & tick prevention. The Guardian Home agrees to pay for all pet-related routine care (heartworm prevention, vaccines, flea & tick medication, food, and grooming expenses).2. The Guardian Family commits to drop off this dog within 48 hours of her showing signs that she's going into heat (bleeding, swollen vulva) and pick up within 48 hours of the breeding. If the guardian family hinders the breeding process in any way (i.e. lying about heat cycles, spaying without consent, or moving without updating Teddy Puppies the Guardian family will be fined the value of the breeding dog, up to is $30,000).3. The Guardian family agrees to drop off the dog within 1 week of her due date (when pregnant) and pick her up when her puppies reach 7 weeks of age.2. The Guardian Family agrees to follow the recommended care guidelines Teddy Puppies gives, keep insurance active for the dog, give daily vitamins when pregnant and won’t take the dog to any densely populated areas with other dogs to prevent illness and the risk of miscarriage (like a dog park). The guardian family commits not to leave a dog in a car when it is over 75 degrees outside.3. The Guardian Family agrees to house their dog indoors and notify the breeder of a change of address or phone number and will remain reachable (respond to important communications within 24 hours).4. The Guardian Family agrees to communicate with Teddy Puppies at least bi-annually with updates. This includes communicating with Teddy Puppies any signs of heat (for example, the first day a female is bleeding or has a swollen vulva), veterinarian visits, weight/height, temperament, obedience training successes, and providing pictures. In case of an emergency or the dog is lost, the guardian family commits to contact Teddy Puppies immediately. 5. The Guardian Home agrees that he/she understands this contract fully and that this contract is under the jurisdiction of the State of Georgia, in the county of Cherokee. Teddy Puppies and the Guardian Home agree that this writing represents the entire agreement between them and that no other representations, either written or oral have been made regarding the dog.6. If the breeder finds it difficult to reach the guardian family via phone/text messages that makes this working relationship hostile and/or if the guardian family refuses to do anything the breeder asks as set forth in this document, the dog will be removed from the guardian home without question and returned to the owner/breeder.7. The guardian family commits to have all pet sitters watch this dog in their home, unless otherwise approved by the breeder.
