-I fully understand and agree that during the staff selelction my application may be reviewed by the School Administration and Board Members other that those to whom it was origianlly addressed. Moreover, I fully understand, if hired, my application materials will become part of my permanent record and that any misstatment of material facts shall constitute ground for discharge.

-I recognize that LSA will conduct background investigations on all employees upon hire.

-I also give my permission to the School Adminsitration to consult with references and other civil authorities in order to verify infomration submitted in this application.

-I understand that this application will be considered active for twelve (12) months from the date filed. I further understand that if I am employed by Lifespring Academy, this application and associated documents will become part of my permanent record.

-I understand that this application is not a contract of employment. In accepting the position, if hired, . I acknowledge that the State of NC may discharge an at-will employee at any time for any legal reason or no reason at all.
