Canine ACE Medical Release Form
At Canine ACE, our top priority is providing a safe and healthy environment where your pet(s) will receive the best possible care. While we ask you to provide us with any medical issues your pet(s) may have, some incidents may occur that we cannot prevent. In the case of a medical emergency, Canine ACE staff will contact you, the pet(s) owner, as well as the closest available veterinary facility for immediate treatment. This may mean we will not contact your primary veterinary office. Our top priority in this case is getting your pet(s) immediate treatment as quickly as possible.

I understand by signing this form, I cannot hold Canine ACE or its owner, employees, affiliates, or guests responsible for any injuries or medical issues that may occur while my pet(s) is/are in their care.

Canine ACE Daycare, Overnight Boarding, Enrichment Activities, Enrichment Training & Training Policies
Daycare Assessments:

All new Daycare dogs are required to pass a temperament assessment to join any activities provided at Canine ACE. The assessment is to make sure your pup will be happy and safe in our Playrooms, with other playful & energetic pups!

Canine ACE reserves the right to deny Daycare to any dog deemed unfit for the program.

We are always willing to help your dog(s) integrate into our Daycare group, if we feel comfortable & safe doing so. Some of the following issues that may result in denial of Social Play may include but are not limited to: Aggressive behavior towards other dogs or humans, possessive behavior, excessive medical issues, extreme anxiety around staff or other dogs. Social Play Temperament Evaluations are by appointment only.

Your dog is always learning from the interactions that they have, be it from other Dogs, their environment, or from the Human handlers that care for them. Canine ACE is committed to helping them learn the right things and gain the right skills while in our care.

Overnight Boarding:

We offer No Kennel Overnight Boarding for dogs who attend Social Play. That means that your dog(s) will spend the day at Daycare, and after hours (night), they'll get to free roam the facility with other dogs and humans. It's basically a Sleepover at their Home away from Home. We have staff on-site 24 hours a day 7 days a week, your dog(s) will never be left unattended.

Enrichment Training / Activities:

Enrichment Training & Enrichment Activities are Add-Ons services for when your dog is Boarding with us or attending Daycare. Some dogs need a little more attention & engagement than just Social Play! With these add-ons, your dog will be offered 20-60 minutes of one-on-one stimulating & satiating activities/training, with one of our well- trained staff members. These activities/training include but are not limited to: Licky Mats, Ice treats, Ball Play, Tug Play, Scavenger Hunt, Flirt Pole Play, Scent Work, Trick Training, Spring Pole Play, Slat Mill Play, Relaxation Massage and Park Adventures! We love to teach skills through Play, and letting Dogs express their ''Dogness''.
Vaccines, Flea/Ticks, Heartworm Medication, Spay/Neuter and Age:

We want your pup to succeed & be safe while in our care! For that reason, we have a few requirements before we can welcome your pet(s) into our facility and our community. These requirements must be completed before scheduling any reservations.

Vaccines, Flea/Ticks and Heartworm Medication:
Vaccines keep your pet and other pets around them safe! We do require ALL pups in our facility to be fully vaccinated with the following vaccines before scheduling any reservations. Bordetella, Distemper (DH2PP, DAPP, DA2PP-L), K9 Influenza, Leptospirosis, and Rabies. We recommend that you use a flea & tick prevention protocol and heartworm preventative medication for your dog.

Age, Spay/Neuter Requirements:
We understand you might want to start socializing your dog(s) at a young age, but we recommend that your dog is at least 6 months old to attend Daycare! Please use these 6 months to learn about your dog's personality, what they like and don't like, we are happy to give advice with our 1-on-1 sessions!
*All dogs must be spayed or neutered by the age of 12 months.

Payment:

Payment is required at the time service is rendered. We accept Visa, MasterCard, Discover, and American Express. You may be asked to leave a card on file.

Late fees:

Daycare or Overnight Boarding guests not picked up by 6:00pm Monday - Friday will be considered overnight guests and will be charged the appropriate boarding fee. Overnight Boarding guests not picked up by 5:00pm Saturday or Sunday will have to stay the night and will be charged the appropriate boarding fee.

Boarding pets who are not picked up by 11:00am on the last day of boarding will be charged a $40 fee. Late fees can be covered with a Daycare Pass.
*Unlimited Daycare Memberships can't be used for this purpose.

Canine ACE Hold Harmless Release
At Canine ACE, every dog is carefully screened for temperament to help ensure a safe and positive environment for all our guests. We do not accept dogs with known aggressive behavior and closely monitor all interactions.

However, Canine ACE cannot guarantee the behavior of every dog in our care. As with any group play setting, there is always a risk -however small- of injury or, in rare cases, death due to but not limited to aggression from other dogs.

By signing this form, you agree not to hold Canine ACE, its owner, employees, affiliates or guests liable or sue for any injuries to and/or death of your dog(s) while on the premises for recreation, daycare, boarding, training and/or in the general care of Canine ACE.

By signing this form below, you are acknowledging and agreeing to the terms put forth here by Canine ACE.
I acknowledge that participation in Daycare and any other activities involves inherent risks, including but not limited to the risk of injury or death to my dog(s). I voluntarily accept all such risks. By submitting this form, I hereby release and hold harmless Canine ACE, its owner, employees, affiliates and guests, from any and all liability for injury, illness, or death of my dog(s) that may occur on or off the premises of Canine ACE - PLAY Centre.

I acknowledge that by boarding or enrolling my pet -who is of advanced age and/or has preexisting medical conditions- with Canine ACE, I assume all associated risks. I agree to release and hold harmless Canine ACE, its owners, employees, and affiliates from any liability for injury, illness, or death that may occur to my pet while in the facility and general care.

I understand that while the epoxy flooring and artificial turf at Canine ACE provides a safe and suitable surface for play, it may occasionally cause blisters, abrasions or scrapes on the pads of my dog(s)' feet. I also acknowledge that some dogs may have heightened sensitivity or allergic reactions to the cleaning agents used throughout the facility, I further understand that minor injuries such as abrasions, nicks, scratches, and saliva residue are a normal result of dog play and interaction. I agree not to hold Canine ACE, its owner, employees, affiliates, or guests, liable for such incidental injuries or any other injuries not stated in this form.

I understand that my dog will be required to go outdoors for bathroom breaks, including during the summer months. While occasional accidents may occur in the playroom or kennels, Canine ACE provides designated, shaded outdoor potty areas, and dogs are never left outside unattended or for extended periods of time. All dogs participating in Daycare, Overnight Boarding, Enrichment Training or Enrichment Activities are required to spend a brief period outside for potty breaks. In cases of extreme weather, the duration of outdoor time may be shortened for the safety and well-being of all dogs.

In the event that my dog becomes ill, injured, or requires veterinary care while in the care of Canine ACE, its owner, employees, affiliates and/or guests, I authorize Canine ACE to consult with my designated veterinarian. I understand that Canine ACE will make a reasonable effort to transport my dog to my designated veterinarian or to an appropriate emergency veterinary clinic if necessary. If my designated veterinarian is unavailable, I authorize Canine ACE to seek treatment from an alternative licensed veterinary provider at their discretion, based on the urgency and nature of my dog's condition. I acknowledge that I am fully responsible for all costs incurred with bringing my pet to and from the veterinary facility, and for any medical treatment and/or bills resulting from it.

I understand and agree that my dog(s) must be brought into and taken out of the facility on a secure leash at all times. I acknowledge that Canine ACE may transport my dog off-site, including to local parks or designated outdoor areas, as part of its services. While reasonable precautions will be taken, I agree not to hold Canine ACE, its owner, employees, affiliates or guests, liable in the event that my dog becomes loose or lost while off the facility premises.

I understand that my dog may be temporarily suspended from Daycare at any time if their behavior becomes disruptive or poses a concern within the Daycare group. Should this occur, a behavioral reevaluation may be required before my dog can return to the program. I also understand that Daycare is designed to be an enjoyable and enriching environment for all participating dogs, so signs of extreme anxiety or inappropriate behaviors — such as snapping, mounting, or overly aggressive play— may result in my dog being permanently removed from the group, for their own safety and the safety of others. Canine ACE reserves the right to make this decision and will communicate with me to discuss appropriate next steps and alternative care options.

I hereby affirm that, to the best of my knowledge, my dog(s) has no history of aggression toward other animals or people. In the event that my dog displays aggressive behavior while in the care of Canine ACE, its owner, employees, affiliates or guests, I agree to accept full financial responsibility for any and all resulting expenses. This includes, but is not limited to, veterinary care for injuries sustained by my dog or other dogs, medical treatment for injured individuals, and the repair or replacement of any damaged property or equipment.
