I, the undersigned parent/guardian, hereby give permission for my child, to participate in Delson Elite League. I understand that participating in basketball involves physical activity and the risk of injury, including but not limited to sprains, strains, fractures, and other injuries.
I acknowledge that it is my responsibility to ensure that my child is physically fit and capable of participating in basketball. I also agree to inform the organizers of any medical conditions or special considerations that may affect my child's ability to participate safely.
In consideration of my child's participation, I agree to the following:
Release of Liability: I, on behalf of myself, my child, and our respective heirs, executors, and administrators, hereby release, indemnify, and hold harmless Delson Elite League, its directors, officers, employees, volunteers, and agents from any and all claims, actions, or damages arising out of or related to my child’s participation in the Youth Basketball Program, including but not limited to any injuries, accidents, or illnesses.
Assumption of Risk: I understand and acknowledge the inherent risks associated with sports activities, including basketball, and voluntarily assume those risks.
Emergency Medical Authorization: In the event of an emergency, I authorize the staff of Delson Elite League to seek medical attention for my child and agree to be responsible for any medical expenses incurred.
Insurance: I acknowledge that I have insurance coverage or will bear the costs of medical expenses arising from any injuries or accidents during my child's participation.
Photography and Video: I consent to the use of my child’s image and likeness in promotional materials, including but not limited to photographs, videos, and social media, related to the Youth Basketball Program.
By selecting "I AGREE", I acknowledge that I have read and understood this waiver and release, and I voluntarily agree to its terms.
