    *After you complete all sections of the form and click on the “submit” button, you will be taken to a screen to make your church/group’s first payment (monthly or in its entirety). Your payment finalizes your church’s registration. For this reason, please be prepared with your credit card or bank account information when you click to submit the completed form.

    *You will receive an electronic confirmation of your registration payment for your records.

    *You will also receive an electronic copy of this completed application form as you submitted it.

    • Why the DreamTables Training? 
    • Having breakthrough prayer already actively underway together in your setting is a prerequisite for registering for a DreamTables cohort. If this hasn’t happened yet, first you’ll need to get it started with implementing 28 days of breakthrough prayer together through the corporate use of Dynamite Prayer (www.dynamiteprayer.com) — or else, gain step by step instructions in the resource Open Road: Adventures of a Breakthrough Prayer Initiative (www.openroadchurch.com).     • YES, we understand that upon acceptance into a DreamTables cohort we will identify, select and assemble a team of 3 or more persons who are committed to attending the DreamTables Cohort trainings with the senior pastor/supervisior, to participate in the coaching sessions, and to partner with others to bring implementation across the entire congregation or ministry.

    • The final step to confirm participation is making the first payment—either for the first installment of $225 (and second installment will be billed 30 days later), or its entirety ($450).
