Service Agreement Terms

Tillwell herein referred to as "service provider"

 

Payment and Authorization Requirements

This service agreement may be completed in one of two circumstances:

1. At-Need Services: When a death has occurred and direct aquamation services are immediately required
2. Pre-Need Planning: In conjunction with a pre-payment plan purchased through TruStage Insurance and completed with a Tillwell representative

Payment Authorization: Funds may only be transferred for services under the following conditions:

• Direct aquamation services are being sought for an immediate need, OR
• A pre-need plan has been purchased from TruStage Insurance

In accordance with Manitoba legislation (The Prearranged Funeral Services Act, C.C.S.M. c. F200), all pre-need arrangements are processed through TruStage Insurance to ensure proper fund management and consumer protection.

Price Protection for Pre-Need Arrangements

For customers who pre-pay through TruStage Insurance and sign this service agreement, Tillwell will honor the contracted price for a period of five (5) years from the date of signature. After this initial five-year period, pricing will be adjusted to current rates every subsequent five years, providing purchasers with extended price protection.

General Terms and Conditions

1. The undersigned acknowledges that I/we have been informed of all prices for services and merchandise.
2. The purchaser authorizes the service provider to perform the requested services and furnish the merchandise described and any other items incidental to the requested services. The only warranties granted in connection with merchandise are the express written warranties, if any, extended by the manufacturers thereof.
3. Representatives of the service provider have explained the risks inherent in leaving clothing, jewelry or other articles with the service provider, where members of the public are permitted to enter onto the premises. I/we hereby release and forever discharge the service provider, its employees, its agents and representatives from any claims or damages arising from the theft, loss or disappearance of any clothing, jewelry, or any articles whatsoever placed or lost in or about the service provider's facilities.
4. For At-Need Services: The purchaser promises to pay the account due to the service provider in full at the time of the arrangements. For Pre-Need Services: Payment is processed through TruStage Insurance as per the pre-need contract terms.
5. The purchaser represents that they are legally authorized or charged with the responsibility for the arrangements contained in this contract, and further acknowledge that the service provider, its employees, its agents, and representatives are acting in reliance upon this representation.

Services Include:

Direct Aquamation Package - $1,295.00 plus GST ($64.75)

• 24/7 support
• Transportation of the deceased into our care (within 30 kms of Tillwell)
• Refrigeration/storage
• Staff services (Registration of Death and all required paperwork)
• Funeral Director Statement(s) of Death
• Aquamation process
• Basic biodegradable urn
• Delivery of remains (within 30 kms of Tillwell)
• Complimentary online obituary
  • TOTAL: $1,359.75

Additional Notes:

1. Extended distance transportation beyond 30 kms billed at $1.83/km (one way)
2. If the deceased is in excess of 300lbs, but not over 500lbs, a surcharge of $495.00 will be applied to the above stated costs
   1. For all bariatric persons, Tillwell will require consultation with the appropriate medical professional (examiner, physician, etc.) to determine suitability for the alkaline hydrolysis unit, which accommodates individuals up to a maximum if 7' in height and 500lbs in weight

Note

These prices do not include any merchandise such as: Urn upgrades, keepsakes or otherwise memorial items, off-site services (including but not limited to: church/social-hall/hotel rentals, flowers, celebrations of life, vigils, celebrant services, cards and invitations, graveside services, and/or any other off-site services overseen by a Tillwell representative and/or another funeral home or director) nor any third party/cash disbursement costs (food, flowers, celebrant services, etc.).

If the deceased is in excess of 300lbs, but not over 500lbs, a surcharge of $495.00 will be applied to the above stated costs. For all bariatric persons, Tillwell will require consultation with the appropriate medical professional (examiner, physician, etc.) to determine suitability for the alkaline hydrolysis unit, which maximally accommodates individuals up to 7' in height and 500lbs in weight.

Any other out of town transfer kms that are not stated within our Aquamation Packages will be billed at $1.83 per km (one way).

The undersigned has fully read the terms and conditions of this service agreement and acknowledges that any other third-party costs will be invoiced separately.

Charges are only for those supplies and services that are used or provided. If we are required by law to use any supplies or provide any services, we will explain the reasons in writing below."

Except in cases where the law requires embalming, you do not have to pay for embalming that you did not order. If we charge you for embalming, we will explain the reasons in writing below.
