BY SELECTING SUBMIT I CONSENT I have read and fully understand the above information. I have had the opportunity to ask questions and by which I have received an answer to my satisfaction. I understand the risks in the administering of a vaccine and I assume the said risks and I hereby give my consent for having my pet vaccinated by the veterinarian or veterinary staff. I shall report to the clinic any complications that might arise following the vaccination. I understand that any additional treatment may incur expense on my part. I understand that my pet(s) have been examined to determine the appropriateness of the immunizations selected. This is not a full and complete physical examination. I recognize the inherent risks of bringing my pet to an outdoor vaccination clinic. Pet(s) may have an adverse reaction to any vaccine. I agree to indemnify and hold Low Cost Pet Spot or any affiliate harmless from and against any and all liability arising out of procedures performed and of my visit to the mobile vaccination clinic today including, but not limited to: loss, injury or death. I also hold Low Cost Pet Spot blameless should I be harmed by my pet or any other pet attending the mobile vaccination clinic. I authorize the following pet(s) to be vaccinated by Low Cost Pet Spot.
Emergency
We provide a low cost alternative for vaccinations, heartworm testing and preventatives. An annual examination with your regular veterinarian is always recommended and in the best interest of your pet’s health. An allergic reaction to vaccines can include hives, facial swelling, nausea, vomiting or general malaise. A vaccine reaction can be immediate or take up to four days to develop. If your pet is having a reaction more severe than some general malaise or soreness, you should immediately seek medical attention with your regular veterinarian. If it is after hours, it is prudent to consult the local emergency clinic. Call your nearest emergency clinic in case of emergency vaccine reaction.
