  The undersigned fully understands that my/my child's participation in the above events/classes exposes my child or me to the risk of personal injury, death, communicable diseases, illnesses, viruses, or property damage. I hereby acknowledge that participation in these events/classes is voluntary and agree to assume any such risks. I hereby release, discharge and agree not to sue the Golden Dragon USA or its officers, employees or agents, for any injury, death or damage to or loss of personal property arising out of, or in connection with, my participation in the event/class from whatever cause, including the active or passive negligence of Golden Dragon USA or its officers, employees or agents, or any other participants in the event/class. The parties to this agreement understand that this document is not intended to release any party from any act or omission of “gross negligence,” as that term is used in applicable case law and/or statutory provision. Further, in consideration for being permitted to participate in these events/classes, I hereby agree, for myself, my heirs, administrators, executors and assigns, that I shall indemnify and hold harmless the Golden Dragon USA from any and all claims, demands, actions or suits arising out of the connection with my/my child's participation in these events/classes. I have carefully read this Release, Hold Harmless, and Agreement Not to Sue and fully understand its contents. I am aware that it is a full release of all liability and sign it on my own free will.

  Waiver and Release of Liability Form

By clicking on this form you confirm that you have read and agree to all terms in the release and waiver. 

I give permission for my child to attend the above events/classes. I understand that personal injury can and may occur to my child, and I hereby authorize Golden Dragon USA, its officers, employees, volunteers and/or agents, to seek and consent to emergency medical attention for my child as needed; and I further agree to be liable for and to pay all costs incurred in connection with such medical attention. I hereby release Golden Dragon USA, its officers, employees, agents and volunteers, from any and all liability, claims, demands, causes of action and possible causes of action whatsoever arising out of or related to any loss, damage or injury (including death) that may be sustained by my child while participating in or traveling to and from this event. I give permission for my child to ride in any vehicle designated by Golden Dragon USA, its employees and adult volunteers, while participating in and traveling to and from this event. I agree to accept full responsibility, financially or otherwise, for any damage my child may do to the property of Golden Dragon USA,, properties visited on outing, other’s personal property, or vehicles used for transportation.

FEES: I understand that I will have to pay $$ upon arrival Please note, there is a non refundable student enrollment fee for each school year of $200 and material fee of $150 (Your child’s spot in the classroom will be held with the signed return of this form and the enrollment fee and material fee.)

REFUND INFORMATION

There are no credits or refunds for class absence or dropped classes. If your child will not attend on a certain day, no credit will be given.

I give my permission for my child's photograph or video to be taken for use by the HongWu Golden Dragon Academy in program brochures, annual reports, website, social media sites and other promotional materials and for release to local Newspapers."
