TRY IT OUT SPORTS LIABILITY WAIVER AND RELEASE FORM
ACKNOWLEDGMENT OF RISK AND RELEASE OF LIABILITY
I, the undersigned, as the parent or legal guardian of the above-named participant (hereinafter referred to as “Participant”), hereby acknowledge and agree to the following terms regarding the participation of the Participant in the Try It Out Sports program organized by Try It Out Sports, founded by Mike Vukovics (hereinafter referred to as “Organization”).
Assumption of Risk
I understand that participation in sports activities carries inherent risks, including but not limited to falls, collisions, and other physical injuries. I voluntarily assume all risks associated with participation in the Try It Out Sports program.
Medical Authorization
I certify that the Participant is physically capable of participating in sports activities and does not have any medical conditions that would prevent safe participation.
In the event of an emergency, I authorize Try It Out Sports and its representatives to obtain medical treatment for the Participant if I cannot be reached. I agree to assume full financial responsibility for any medical services provided.
Release and Waiver of Liability
I, on behalf of myself and the Participant, hereby release, waive, and discharge Try It Out Sports, its founder, employees, and affiliates from any and all claims, liabilities, damages, or causes of action arising out of or related to the Participant’s involvement in the program, including but not limited to personal injury, illness, or property damage.
Indemnification
I agree to indemnify and hold harmless Try It Out Sports, its founder, employees, contractors, volunteers, and affiliates from any claims arising from the Participant’s participation in the program.
Photo/Video Release
I grant permission for Try It Out Sports to take photographs and videos of the Participant during program activities for use in promotional materials, including but not limited to social media, brochures, and the official website. I understand that no personal identifying information will be disclosed without additional consent.
Code of Conduct
I understand that the Participant is expected to follow all program rules and respect coaches, staff, and fellow participants. Failure to comply may result in removal from the program without refund.
Refund Policy
I acknowledge that Try It Out Sports maintains a no-refund policy, except in cases of program cancellation by the Organization.
