    This submission portal is only for successful applicants with a FINAL offer of admission for a degree program in the University of the Philippines Cebu for the First Semester AY 2025-2026. 

    IMPORTANT. Before you fill this form, please read the First Year Registration Guide for the full list of registration requirements. As specified in the guide under "Online Copy", the following documents should be ready to upload as attachments:

    1. UPCAT Offer of Admission or Notice of Qualification for Admission
    2. 2x2 color ID, for your school ID
    3. High School Report Card, Form 138, or Transcript of Records (if transfering from outside UP)
    4. PSA Birth Certificate
    5. Duly accomplished Student Directory Form
    6. Voluntary opt-out form, if opting out from free college tuition

  • Requirements

  • How did you qualify for UP? Choose the option that best fits how you are applying.

    • UPCAT Qualifier for UP Cebu: You passed the UPCAT (University of the Philippines College Admission Test) and have been accepted into a degree program at UP Cebu. This is the usual way students enter UP after high school.
       
    • Direct Appeal: You were not initially admitted through the regular UPCAT process, but you submitted a formal request for admission directly to UP Cebu. Your application was reviewed by the college or program and was approved based on available slots, your qualifications, and other considerations.
       
    • Iskolar ng Bayan: You qualified under the Iskolar ng Bayan Act (RA 10648) as a graduate from a public high school, ranked top 10 in your class, and you have not enrolled in college before. You applied through the DIWA portal for your UP campus (must match your region/school), met the UPG cutoff, and were admitted based on program quotas and review by the campus scholarship committee.
       
    • VAAS Athlete: You are applying as a student-athlete under the Varsity Athletic Admission System (VAAS). This means you're joining UP Cebu through your athletic skills, not through UPCAT.
       
    • High School Graduate (for the Fine Arts Certificate Program): You are a high school graduate who wants to take the Certificate in Fine Arts course. You have not taken any college subjects in UP or in any other university.
       
    • Transferee: You have already studied in college, either at another UP campus or a different school or university. You are now applying to continue your college studies at UP Cebu.

  • Important Instructions Regarding Your Birth Certificate

    1. Unreadable Name on Certificate: If your name on the certificate cannot be clearly read, please attach a copy issued by the Local Civil Registrar.
       
    2. Name Mismatch: If the name on your report card is different from the name on your birth certificate, the name on your birth certificate will be considered official.
      • In case of a discrepancy, you must also submit a Notarized Affidavit signed by two disinterested persons.
         
    3. Scanned Copy Unavailable? If you're unable to upload a scanned copy of your PSA Birth Certificate at this time, please upload a photo or screenshot of your written explanation stating why you cannot submit it. This will temporarily serve in place of the actual birth certificate in the online portal.

    NOTE: You are expected to submit the original PSA Birth Certificate upon registration. This is non-negotiable.

  • Note: As a varsity scholar or student enrolled in the Associate of Arts in Sports Science (AASS) program, you must keep at least one personal copy of your birth certificate, since you will need to submit copies from time to time, such as during sports registrations.

    The copy you give to the Office of the University Registrar (OUR) will be kept by their office and cannot be given back. .

  • Declaration of Voluntary Opt-Out from Free Tuition Privilege

    I am voluntarily opting out of the Free Higher Education privilege provided under Republic Act No. 10931, also known as the Universal Access to Quality Tertiary Education Act of 2017, effective this semester and for all subsequent semesters, unless I submit a formal request to revoke this decision.

    By making this declaration, I understand that I will pay the full tuition and other applicable fees beginning this semester, as my continuing contribution to the University.

    I am uploading a signed Voluntary Opt-Out Form, confirming my decision to waive the Free Higher Education privilege.

  • #4: National Service Training Program Preferences

  • Please select your preferred NSTP component and indicate your preferred class schedule(s) in order of priority. Final sectioning will depend on the schedule of your Major and GE courses.

    There are two NSTP components options:

    1. CWTS (Community Welfare Training Service) is a community-based program that promote general welfare and improve quality of life, focusing on areas such as health, education, environment, safety, entrepreneurship, and civic values.
    2. ROTC (Reserve Officers' Training Corps) is a military training program under R.A. 7077 designed to prepare students for national defense service through discipline, leadership, and physical readiness.

    You must complete both NSTP 1 and 2 under the same component. For example:

    • If you took CWTS 1, you must also take CWTS 2.
    • If you took ROTC 1, you must also take ROTC 2.
  • The following NSTP schedules are available. Please rank the schedules listed below by dragging them into your preferred order, with your number one choice at the top.

  • #5: Physical Education Classes

  • The following PE classes are available. Please rank the classes listed below by dragging them into your preferred order, with your number one choice at the top.

    • Your PE class will be assigned based on your ranked preferences and your available schedule, as determined your major and GE courses.
    • To rearrange the list, click/press and hold, then drag each item to its desired position.

    Ensure that all options are ranked in order of preference before submitting.

  • #6: Internet Connectivity

    • Limited or Unstable Internet: Can only support basic messaging and occasional browsing. Video streaming (YouTube, TikTok, Zoom, etc.) is mostly not usable or frequently interrupted.
    • Moderate-Speed Internet: Supports basic video streaming at low to standard quality (144p–480p). Video calls are possible but may lag or drop.
    • Fast and Stable Internet: Supports smooth video streaming at high quality (720p or higher), reliable video calls, and online class participation with minimal interruptions.

    Please indicate whether you allow UP Cebu to share your grades and other student information with your parent(s) or legal guardians. This consent remains in effect unless you withdraw it by writing to the Office of the University Registrar.

  • By choosing not to authorize us, please understand:

    1. We will not be able to speak to your parents about your grades, enrollment, or school standing.
    2. This may cause difficulties if your parents or guardians try to help you with school matters.
