This request does not guarantee your dates. You will be notified witnin 48 hours of availability. You will be required to pay a 25% deposit of the entire boarding cost along with Pet Information, Medical Waiver, and Boarding Contract wihin 5 days of the submission date of this form. This deposit will, of course, be applied to your total boarding fee. BE SURE TO READ THE PROCESS AND REQUIREMENTS FOR BOARDING BEFORE YOU SUBMIT THIS FORM.
