I understand that falsification of this application will result in immediate discontinuance of service.I understand that the application must be filled out completely.I understand the utility is not responsible for any water damage to a home or business.I understand a four-hour window for an appointment will be set to turn service on. If I am not there, I will be charged an additional service fee for the second call.I understand half of my unpaid balance will be added to my account to cover collection fees or court costs in the event I am turned over for collections.I understand that if the utility waives my deposit but I do not keep my bill current, a deposit may be requires at a later date.
