Boarding Contract

This boarding contract is standard for a boarding facility. Please know that we take our job of caring for your pet(s) seriously and always put their safety first. As caregivers, we treat your pet(s) as if they were our own and do our very best to care for them.
PLEASE NOTE: If your pets do not go to the same veterinary clinic, you will need to submit a form for each pet.

Emergency Veterinarian Treatment
1. All representative's of Gregory's Gift of Hope Rescue agree to exercise all due and reasonable care to prevent injury or illness to my pet during its boarding period. Should either occur, I release Gregory's Gift of Hope Rescue, it's representatives, the owner(s), and employees of the facility and they shall not be held personally liable for such injury or illness.*
2. Should there be a need for emergency veterinarian care, Gregory's Gift of Hope Rescue will contact the veterinary clinic listed on this form during regular business hours. If the need for emergency care is after hours, Gregory's Gift of Hope Rescue's authorized personnel will contact the emergency vet on call for direction. If the facility veterinarian is unable to administer treatment, your pet will be taken to either Animal Emergency & Referral Center (AERC) in Oakdale, MN or Veterinary Emergency Group in Woodbury, MN. for care. I, as the owner of said pet or the representative of the owner of said pet, will be responsible for all incurred charges related the emergency care of my pet. Gregory's Gift of Hope Rescue will attempt to notify me once the emergency has been assessed but will begin care first. I understand the veterinarian will begin emergency care and assessment of the situation, begin treatment, and will then notify me to continue care as I would like. I understand this policy and procedure.*
3. I agree to pay for all veterinarian services incurred for my pet's stay should emergency serves be required.*
5. Gregory's Gift of Hope specifically requires all bet be vaccinated against communicable diseases prior to boarding. Despite these precautions, I acknowledge that my pet(s) will be in an environment with other pets during boarding and understand that any pet may unknowingly harbor and spread a communicable disease. *
6. I release Gregory's Gift of Hope Inc, and I waive all claims and liability against Gregory's Gift of Hope for, all losses, damages, costs, and expenses arising out of or in connection with any communicable disease contracted by my pet(s) during boarding.*

7. I understand that the boarding fee is charged on the day of arrival regardless of check in time and there is a full day charge for the day of departure if check out is after 10:00 AM.*
8. I agree to pay all charges from the day of drop off to the day of pick up and understand that my pet(s) will not be released to me until all said charges are paid in full.*
9. The balance of my pet(s) boarding fee is payable at the time my pet(s) is picked up. Cash or card payment only. *
10. I understand that reservations can be extended only in case of advanced notice Gregory's Gift of Hope's approval, and availability.*
11. I agree to any or all reasonable costs for any property damage or personal injury caused by my pet during it's stay.*
12. I understand that any pet(s) left for ten (10) days beyond the agreed date of pick or refuses to pay agreed upon charges, pet(s) will be deemed abandoned. The pet(s) will then become the property of Gregory's Gift of Hope Rescue and a surrender fee of $200 will be charged in addition to the boarding costs incurred.*
13. Should it occur, I release and waive all claims and liability against Gregory's Gift of Hope Inc, it's representatives, the owner(s), and staff for all losses, damages, costs, and expenses arising out of or in connection with any injury or illness to my pet during the boarding period. *

To secure your reservation there is a deposit of 25% of the total boarding fees plus a signed Contract/Medical Waiver and Pet Information form within 5 days of your Date Request submission date.
