Badge Request Form for Courts only
This Form will be used to request the badges required for entrance to the courts. Please follow the steps below to complete.

There are a few reasons why you might be requesting a badge.

Please see the list below and follow the appropriate steps.

1) You are new to the neighborhood or have lived here for a while and have never had a badge before. Start at owner name and complete fields for all members of your household that need a badge. You do not need to answer the multiple choice questions.

2) You have lost your existing badge and need a replacement enter complete your information in the Owner Name field and select under I need a replacement badge if you would like to use your existing photo or have uploaded a new one.

3) You have a child that has aged into a new age bracket or now needs a badge. Complete your owner information as the parent or guardian of the child, select need child aged into new bracket, select the new bracket and add a current photo.

Court badges are not avilable to Collington East residents.
