Registration is $100 per athlete. This includes team uniform t-shirt, league registration fees, awards, and an end of the season banquet.

The season will begin with practice on Tuesday, August 5th. Practices will be on Tuesday's and Thursday's, from 6-7:15/30pm, through the season at Streetsboro City Park. Meets will be on Sunday afternoons.

Release of Liability

Running Rockets, Inc. Liability Waiver

In consideration of being allowed to participate in any program offered by Running Rockets, Inc., related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Running Rockets, Inc., Administrators, Coaches and Volunteers, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Medical Forms Waiver

Since Running Rockets is a youth program, parents/guardians usually attend practices/competitions. With this being said, Running Rockets, Inc. is doing without requiring you to fill out a medical form. In the event of an injury, all attempts will be made to treat the injury on scene/contact the parents prior to any further medical attention. However, if any Running Rockets, Inc. coach, director, or volunteer deems necessary to contact emergency services, Running Rockets, Inc. will not be held liable for the medical expenses. Also, you agree that you are accepting risk of participating in any Running Rockets, Inc. activity given any prior medical history that may flare up due to these activities.

Pre-Participation Evaluation Waiver

Pre-participation evaluations, or physicals, are required by schools prior to participating in athletics starting usually in middle school. Running Rockets, Inc. does not require a physical evaluation prior to participating. However, if you have a family history of certain medical conditions, it is highly recommended to visit a doctor prior to the season. A copy of this form can be found on the forms tab of the homepage.

By agreeing to this waiver, you understand that a physcial is not required, but is highly recommended. Also, you understand that if a medical condition does occur from participating in any Running Rockets, Inc. activities, Running Rockets, Inc. cannot be held responsible for that medical condition and its accompanying expenses.

Photographic Release

I grant permission to Running Rockets, Inc. to use photographs and/or videos of my child/ward that may be taken during the season for publication on the website and or other media.
