Hearth & Home Termite & Pest LLC agrees to provide pest control service at {serviceAddress}.
Client agrees to make the place of service available for the treatment and/or inspection as often as necessary to control pest(s) selected above.
Technicians are only able to conduct service in affected areas that are visible, accessible and unobstructed at time of service. Services will not be conducted in areas such as, but not limited to, those that are enclosed or inaccessible, areas concealed by wall coverings, floor coverings, ceilings, furniture, equipment, appliances, stored articles, or any portion of the structure in which services would necessitate removing or defacing any part of the structure(s).
This pest control service does NOT include service for, nor does it provide for damages arising from, infestation of termites or other wood destroying insects.
Client waves all claims for damages to property or persons which may results indirectly from work performed by Hearth & Home Termite & Pest LLC, with the exception of gross negligence on the part of said company and/or its representative(s). This agreement does not provide for the repair of present or future damages to the service address, nor does it provide reimbursement for repair expenses allegedly arising from pest infestations prior to, or after, services.
Client agrees to pay Hearth & Home Termite & Pest LLC invoices upon receipt. Hearth & Home Termite & Pest LLC reserves the right to terminate any scheduled services if payment is not received within thirty (30) days of invoice.
Client signature below provides Hearth & Home Termite & Pest LLC and its representative(s) permission to provide pest control services on the property at {serviceAddress}.
