    Please submit the form twice if you are signing up 2+ children. Please read the following carefully and take your time. If you do not wish to complete this form you will not be accepted into the program. This waiver is valid from registration and also after the participant's departure from the program.
  • Affirmation of Identity and Legal Compliance

  • By signing this e-document, I, the undersigned, affirm that the information I have provided, including my name and the involved children's name is true, accurate, and complete to the best of my knowledge. I acknowledge that failure in complying with any terms, conditions, or rules can result in termination from the program or legal action. I acknowledge that any false statements, misrepresentation, or intentional omission of information may result in legal consequences, including but not limited to termination from the Program and potential legal action. I understand and agree that any misrepresentation could result in penalties under applicable law.

  • TYA Terms and Conditions

  • By signing this e-signature you agree to the following,
    Respect: You will not use any verbal slurs, forms of hate speech, or verbally or physically fight any coaches, parents, kids, or spectators. Disrespectful behavior will not be tolerated.
    Refund Policy: You will not ask for or obtain a refund if your child is absent from class due to personal reasons like sickness, busy schedules, or other commitments. Refunds will only be considered if your child attended 0 classes within the month or if every single class was cancelled due to weather.
    Class Make-Up: We are only responsible for making up classes cancelled due to weather conditions. Absences due to personal reasons cannot be made up or refunded. On the event that a make-up class occurs it is not our fault if you do not appear. If you do not appear to the make-up class that is on you and you will not ask for a refund or a replacement make-up class.
    Payments & Attendance: You must pay on time, by the 1st of every month. We understand most of you are busy with other things so we will accept payments before the 2nd of the given month before 11:59 PM, If you still haven't paid by then it will result in a late fee of $15 being added. If you still have not paid by the 5th of the given month it will result in consequences such as termination from the program. Regarding attendance, by signing up for your child's participation and this form you agree to bring them for all if not most of the sessions in the month. If your child cannot come regularly throughout the month please explain the reason to us as attendance is very important. 
    Program Dismissal: You can be removed from the program for any reason that is justified by the staff or behavior deemed wrong or unjust, even if not explicitly stated in these terms. A staff member or parent must request your dismissal, and the decision will be made accordingly.
    Behavior During Games/Events/Practices: Aggressive behavior or unsportsmanlike conduct at any program event, including in-house games/tournaments, will not be tolerated and may lead to removal from future events or the program. 

    Media Consent: By registering your child(ren) with TYA the Parent/Guardian grants the team the right to capture and use photographs and video of the Participant during all official team activities (games, practices, events) for the reasonable, non-commercial purposes of team promotion on official club websites/social media, educational/training analysis by coaches, and specifically for announcing Player of the Month and other team awards. This consent applies to documentation, media announcements, and digital sharing, with the understanding that the team will not use the Media for external commercial gain, and the Parent/Guardian retains the right to revoke this consent at any time by contacting a staff member.

  • TYA Liability Waiver

  •   By signing this Agreement, the Guardian or Parent acknowledges and agrees that they will not pursue legal action or make any claims against Telfair Youth Academy, its coaches, Haridev Dave and Tanay Kulkarni, their parents, staff, volunteers, other participants, or any associated persons for any injury, harm, or damage sustained by the Participant, whether arising from the risks mentioned above and below or any other incidents related to the Program starting from the date of October 31, 2024 up until February 1, 2100 and the Parent/Guardian e-signing this document agrees to not take legal action even after their departure from the program for incidents that happened during their time in the program. 

    Assumption of Risk: As a parent/guardian you acknowledge that participation in soccer training and associated activities, including but not limited to field play, drills, and games, involves inherent risks, including the risk of physical injury, harm, or even death. The Guardian/Parent voluntarily assumes all such risks, including the risk of injury caused by other participants or the conditions of the playing field.
    Release of Liability: In consideration of the Participant’s participation in the Program, the Guardian/Parent, on behalf of the Participant, hereby waives, releases, and discharges Telfair Youth Academy, its coaches, staff, volunteers, affiliates, and all other associated persons from any and all claims, demands, actions, or causes of action, whether arising in contract, tort, or otherwise, for any injury, harm, or damage sustained by the Participant as a result of their participation in the Program. This release includes, but is not limited to, injuries resulting from field play, interactions with other participants, or any accidents occurring during training sessions or Program activities.
    No Claims Against Other Participants: The Guardian/Parent agrees not to file any claim, lawsuit, or legal action against any other participant, coach, staff member, or volunteer in connection with any injury, harm, or damage sustained by the Participant during the Program. The Guardian/Parent understands that Telfair Youth Academy and its coaches are not responsible for the actions or conduct of other participants, and agrees to waive the right to seek any damages arising from such actions.
    Medical Treatment: The Guardian/Parent grants permission for Telfair Youth Academy to administer or arrange for emergency medical treatment in the event of injury, accident, or illness during Program activities. The Guardian/Parent acknowledges that they are responsible for all medical costs incurred as a result of such treatment.
    Indemnification: The Guardian/Parent agrees to indemnify and hold harmless Telfair Youth Academy, its coaches, staff, volunteers, and any affiliated parties from any and all claims, damages, expenses, or liabilities arising from the Participant's actions, conduct, or participation in the Program, including any legal fees or costs incurred in defending against such claims.
    Acknowledgment of Safety Measures: The Guardian/Parent acknowledges that Telfair Youth Academy takes appropriate safety precautions, provides proper equipment, enforces rules and guidelines, and ensures that all students are properly hydrated and energized during training. However, the Guardian/Parent understands and accepts that accidents and injuries are a common part of physical activities and that Telfair Youth Academy is not responsible for any body external or body internal injuries or damages that occur during participation.
    No Refunds: The Guardian/Parent acknowledges and agrees that no refunds will be provided for Participant absences, including due to illness, scheduling conflicts, or other personal reasons. Refunds will only be considered if the Participant attended a total of zero classes for the entire month or if all classes were canceled due to weather conditions. By digitally signing this form the parent/guardian agrees not to take any action including legal action in attempt to take their money back.
    Program Dismissal: The Parent/Guardian understands that the Participant may be dismissed from the Program for any behavior deemed inappropriate, including, but not limited to, verbal or physical altercations with coaches, other participants, or spectators. Any such dismissal may occur with or without prior warning and is at the sole discretion of the coaches or Program organizers. The Parent/Guardian acknowledges that no refund will be provided if the Participant is dismissed from the Program after payment has been filled for the month.
    Acknowledgment of Understanding: By signing this Agreement, the Parent/Guardian acknowledges that they have read, understood, and agreed to the terms and conditions set forth herein. The Parent/Guardian further acknowledges that no oral representations or inducements have been made to them outside of this Agreement, and that they are signing this waiver with full knowledge of its legal consequences.
    Governing Law: This Agreement shall be governed by and construed in accordance with the laws of Texas in the United States of America, without regard to its conflict of law principles. Any disputes arising out of or related to this Agreement shall be resolved in the appropriate courts located in Sugar Land, TX, USA
