Waiver Form Surdawi Bazzar & Activities Registration 6/28/2025

RELEASE 

I hereby acknowledge that I intend to participate in the activity above of Surdawia Bazzar located at MAS-Columbus. I am aware that this activity has risks of loss, physical injury and even death and I am voluntarily participating in this activity with knowledge of those risks. I hereby release and discharge Surdawia Bazzar and MAS-Columbus, its directors, officers, members, agents representative, employees, instructors, volunteers, successors and assignees from any claim I now have or may hereafter have resulting from my use of the Surdawia Bazzar and MAS-Columbus facilities and equipments. To the fullest extent permitted by law, I agree to defend, indemnify and hold harmless Surdawia Bazzar, its directors, officers, members, agents, representative, employees, instructors, volunteers, successors and assignees, for and against any and all claims, demands, actions, losses and expenses of every kind in any way arising out of my activities under this release. I further acknowledge there are no warranties, express or implied, concerning the facilities, equipment, volunteers or activities at during the Surdawia Bazzar event.

AUTHORIZATION - Furthermore, in case of injury, if I cannot be reached within a reasonable time, I authorize Surdawia Bazzar and MAS-Columbus, and/or its agents or volunteers to obtain necessary medical treatment for my child (if under 18). I also assume the financial responsibility for any medical treatment for my child.

PHOTOGRAPHY - Surdawia Bazzar as part of its event, frequently photographs and video tapes those who participate in during the event. These photos and videos are used for historic documentation as well as for publicity purposes. By my signature, I give permission and full consent for my youth to be photographed or videotaped as part of Surdawia-sponsored or associated events and understand that photos or video of the event may be used in one or more promotional pieces, displayed throughout the building and/or on the web site. This permission may be revoked in writing and submitted to the Executive Director email at surdawia@gmail.com. 

Code of Conduct

Surdawia Bazzar is providing the Bazaar for the community in Columbus. Participation in the organization’s programs is subject to the observance of the organization’s rules and procedures. The activities outlined below are strictly prohibited. Any participant who violates this Code is subject to discipline, up to and including removal from the program.

Participants must refrain from engaging in any type of violence, bullying, harassment, or abusive behavior towards adults, peers, during the Surdawia Bazzar event.

Participants must refrain from the possession or use of alcoholic beverages, illegal drugs, explosives, firearms, weapons or other similar items on during the Surdawia event.

Participants must respect, adhere to the rules of, and follow the instructions given to them by a facilitator or leader of the Surdawia Bazzar Event.

Participants must show kindness and respect to the people around them (their peers, community elders, group leaders, counselors, etc.) and their surrounding environment during the Surdawia Bazzar during the event.

Participants must not associate or be an accomplice with anyone engaged in any type of violence, bullying, harassment, or abusive behavior towards adults and peers during the act.

Participants must report any acts of violence, bullying, cyber-bullying, harassment, or abusive behavior towards adults and peers during the act.
I have read and I understand the Surdawia Bazzar Code of Conduct for the event. I agree to abide by the rules described above and understand that the participant may be removed as a participant if they violate any of these rules.
