    I hereby release and agree to hold Out of the Box Theater harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the theater, or that may otherwise arise in any way in connection with any services received from Out of the Box Theater. I understand that this release discharges Out of the Box Theater from any liability or claim that I, my heirs, or any personal representatives may have against the theater with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Out of the Box Theater.

    2025-2026 School Year
    By registering with Out of the Box Theater, we are committing to provide your student with a high quality educational and performance experience. By signing this commitment letter, you as the parent, agree to be responsible for the monthly payments assigned to the classes your child is registered for.

    PAYMENT RESPONSIBILITY:

    Monthly tuition $60/$50 for additional siblings or multiple classes

    Production Fee for Fall $125 due on first day of class & includes Cast Tshirt

    Production Fee for Spring $165 due on audition weekend....November 21st includes Cast Tshirt

    Withdrawal will need to be made with an email or letter to Amy Egnew, tuition will continue to accrue on your account if a withdrawal notice is not received. 
    The production fees are non-refundable. Tuition is due by the first class of the month. If tuition is not received by 10th of the month an additional $10 late fee will be added. ALL fees and tuition must be paid before a performance or the student will not be allowed to perform. Tuition is not refunded for a missed class. I understand that a return check fee of $35 will be charged in the event a check is returned.
    I understand the above financial obligation and agree to pay the tuition in full.

    November 28 THANKSGIVING BREAK No Classes

                      I have read the calendar and understand that if excessive absences occur during rehearsal or full/partial absences occur during mandatory rehearsal, then my child’s part may be reduced or changed. Excessive absences (besides illness) make it difficult for the entire cast to learn and master their roles. I understand that communicating conflicts as early as possible helps the directors adjust the rehearsal schedule to minimize rehearsal disruptions.

  • YOUTH LIABILITY RELEASE AND WAIVER

    In consideration of being allowed to participate in any way in Enrichment Academy’s Out of the Box Children’s Theatre related events and activities, the undersigned:

    1. Agree that the parent(s) and/or legal guardian(s) of the participant should inspect the facilities and equipment to be used, and if the parent or guardian believes anything is unsafe, he or she should immediately advise supervisor of such condition(s) and refuse to participate
    2. Acknowledge and fully understand that each member/participant may be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
    3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death
    4. Release, waive, discharge and covenant not to sue Enrichment Academy’s Out of the Box Children’s Theatre or its affiliated clubs, their respective administrators, directors, volunteers, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as “releases”, from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.

    I HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT I HAVE GIVEN UP SUBSTATNTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY

    I give Out of the Box Theater permission to publish in print, electronic, or video format the likeness or image of my child and the art/works created by my child. I release all claims against the Out of the Box Theater with respect to copyright ownership and publication including any claim for compensation related to use of the materials.

  • STUDENT CODE OF CONDUCT (please review with student)

    By signing below, I acknowledge that both parent and child agree to the following:

    1. I agree to attend all mandatory rehearsals. 

    2. If I must miss any class/rehearsal for any reason. I will contact the director prior to the beginning of the rehearsal. I will make every effort to tell the director of a conflict as early as possible.

    3.If I miss several rehearsals that are not illness related, my part may be changed.

    4.I agree to wear the costumes, wig, or hairstyle of the director's choosing.

    5.I agree to work as part of a team. Any actor whose attitude or actions are disrespectful will be asked to leave the show.

    6. I understand that participating in shows require collaboration. I agree to contribute my ideas and energy at appropriate times.

    7.I understand the director will make final choices pertaining to my role(s)

    8. I agree to do all that I can to make rehearsals the best they can be.
