Consent and Agreement

In the event that {studentName} is presented for, or requires medical treatment or surgery or any other form of medical care or aid, I, {primaryParentguardian26}, as the parent/legal guardian of the above named student, do hereby authorize the Directors/Chaperones standing in loco parentis to be consulted with, and consent to, any medical treatment or care deemed necessary by any doctor, nurse or other medical personnel. I also guarantee payment of all charges incurred for medical treatment such as, but not limited to physician, hospital, x-ray, lab, drugs, and EMS. (For emergency use only. Every effort will be made to contact parent/guardian prior to treatment). I agree to indemnify and hold harmless the choir directors, staff and chaperones of Stewarts Creek High School Choirs, Stewarts Creek High School, and Rutherford County Schools, its agents and employees for any and all claims, demands, actions, rights of actions and/or judgments by or on behalf of the above named student arising from or on account of said procedures and/or treatment(s) rendered in good faith.

Officially sanctioned events and activities requiring off-campus travel, involve student choir members leaving from, and returning to, Stewarts Creek High School (or other designated locations) at announced times and in an organized fashion will take place each academic year. Travel to officially sanctioned events will primarily occur on school buses driven by Rutherford County School employees or approved operators or on commercial charter buses when applicable.

Student choir members are responsible for abiding by the rules and policies of Stewarts Creek High School Choirs, Stewarts Creek High School and Rutherford County Schools at all activities. All rules and regulations will be clearly stated prior to events. In case of serious violation of any rules or regulations, parents/guardians will be notified by telephone, if possible, and arrangements will be made for the child to return home. Should the above discipline be necessary, parents/guardians will be responsible for any expense incurred.

Not all events and activities will apply to all choir members and there may at times be officially sanctioned events that involve a particular section or particular members of the Stewarts Creek High School Choirs. There may also be times when a small number of choir student members are involved in officially sanctioned events and activities where travel via bus would be impractical. In such cases students may be asked to drive themselves (with parent permission) or ride with an approved parent/guardian driver to and from these activities. Parents/guardians will be notified in advance when such circumstances should arise.

Specific schedules – i.e. “Gig Sheets” will be available the week of each event. Events, activities, times, and locations are subject to change.

By signing below, you are giving your student permission to attend the following, officially sanctioned choir events for the 2025-2026 academic year:

• Fall Choral Retreat (Apollo and Athena Choirs only).  August 4-5, 2025.  Leaving before 9am on Monday, returning after 9pm on Tuesday.  Camp DoReMi in Harstville, TN.
• All-County Choirs (only for students who audition for Mid-State and are placed in the All-County Choirs).  November 3-4, 2025.  5pm-9pm Sunday; 7am-9pm Monday.  FUMC Murfreesboro.
• Mid-State Choirs (only for students who audition for Mid-State and are placed in the Mid-State Choirs). November 17-18, 2025. 4pm-10pm Monday; 7am-9pm Tuesday. Belle Aire Baptist Church, Murfreesboro.
• RSMS Concert (Apollo students only).  December 4, 9, or 11, 2025.  3:30pm-9pm.  Rock Springs Middle School.
• Freshmen Honor Choir (only for students who audition for Mid-State and are placed in the Freshmen Honor Choirs). January 22, 2026 with Snow date of January 29, 2026. 7am to 9pm.  Belle Aire Baptist Church, Murfreesboro.
• MTVA Choral Performance Assessment.  We will travel during the date window of February 24-27, 2026.  Each choir will be given an exact leaving, performance, and return time that will happen one or more of these days with the possibility of arriving as early as 7am and returning as late as 6pm.  FUMC Murfreesboro.
• Run-Out Concert (Athena and Apollo Choirs only).  March 1, 2026.   Arrive at 3pm, concert at 7pm.  Location TBD, but likely a church nearby.  
• ACDA Southern Division Convention (Apollo and Athena Choirs only).  March 4-8, 2026.  Honor Choirs and other performances sometime during the date window; a comprehensive gig sheet will be given with more details.  Memphis, TN.
• All-State Choirs (only for students who audition for Mid-State and are placed in the All-State Choirs). April 9-11, 2026. Leaving at 9am on Thursday, returning late on Saturday evening.  Opryland Hotel.  

 

I attest that I, {primaryParentguardian26}, am a parent or legal guardian of {studentName} and have read and understand the travel requirements of Stewarts Creek High School Choirs, including expected behavior of choir members and remediation if problems arise. I give permission for my child to travel in all modes described herein for all officially sanctioned events listed above and performances of the Stewarts Creek High School Choirs . I furthermore give permission for my child to participate in the above-listed officially sanctioned events of the Stewarts Creek High School Choirs, and that this permission shall remain in effect for the duration of one calendar year from the authorized date on this form.

 

 

One never gets a second chance to make a first impression. 

Performance attire includes a robe (provided by choir), choir t-shirt (provided by choir), black, closed-toe dress shoes (provided by student but approved by directors - flats preferable, no sneakers or boots) and black dress pants (no denim – provided by student but approved by directors) that, when worn without the robe, meet SCHS dress code. If you wear socks (provided by student but approved by directors), they must be solid black.

The choir will provide one black choir t-shirt for the school year.  Only SCHS Choir T-Shirts can be worn for performances (i.e., no other black t-shirts).  Students may wear any black choir shirt provided by the SCHS Choir (including previous year's choir shirts), under a choir robe, but the current year's Choir T-Shirt must be worn for performances where the Choir T-Shirt is visible.

• Families can purchase additional shirts for $8 at the beginning of the school year.
• Emergency shirts can be purchased throughout the year for $16, with windows for purchasing shirts opening about two weeks before a performance.
• Shirts will not be for sale the week of a performance.

 

 

If a student requires us to provide an SCHS Choir T-Shirt on the day of a performance:

• student will perform and receive a maximum grade of 50;
• student can improve that grade by completing Part 2 of the Alternative Assignment for that performance;
• student will be charged $16 for the shirt given.

 

All students issued school-owned choir robe agree to treat them with respect and take measures to avoid loss or damage.  Replacement values of each choir robe is $50.00.

The Stewarts Creek High School Choirs program is a co-curricular activity, with activites in and out of school.  Participation in choir is not a requirement for graduation, but rather is a voluntary and elective choice. 

Because annual funding through the Rutherford County School budget accounts for a small fraction of the total needed for the fundamentals of an effective choir program, funding for about 95% of the overall annual budget is requested as an investment from each family in the form of a financial contribution. We ask that each student in Giovani Voci, Meistersingers, and Bel Canto contribute $65 as a required class fee by the end of August each Fall.  Students in Apollo and Athena should contribute $40 as a required retreat fee each Fall and a $65 required class fee by the end of January each Spring.

No student will ever be denied participation in the Stewarts Creek High School Choirs due to a family’s inability to contribute financially. Fundraisers are setup to help students and families make their contribution.

If there is ever a financial situation that arises, please do not hesitate to contact the choir directors. We are here to help. Communication is the key. To avoid report cards and/or student records/transcripts being held, we ask that all families contact the choir directors to settle all class fee obligations. We must also insist that all financial obligations must be current or paid in full in order for students to participate in optional special trips. 

Students approved for Free/Reduced Lunch or RCS Fee Waiver will have the $65 Choir Class Fee waived; however, other fees (for honor choirs, special trips, etc), cannot be waived and are expected to be paid in full.

I understand that my child's participation in the Stewarts Creek High School Choirs is both a co-curricular activity and elective choice not required for graduation. I agree to satisfy my financial contribution to the Stewarts Creek High School Choirs as outlined above.

The Stewarts Creek High School Choirs function on a high level due to the coordinated efforts of Stewarts Creek Administration, Faculty, and Parents.  We cannot be successful without everyone present at all in-school and after-school choir events.  Be proactive in maintaining a personal calendar with your dates for family, church, sports, choir, etc.  Communicate any potential conflicts that might arise to choir directors, coaches, etc.

All concerts listed below are required of all students:

• Sept 25 - 7pm - Fall Choir Concert (all choir students)
• Oct 28 - 6:30pm - Choral Extravaganza (all choir students)
• Feb 12 - 7pm - Pre-Adjudication Concert (all choir students)
• Apr 19 - 7pm - Choral Masterworks Concert (all choir students)

In addition, the events listed below are required for the indicated students only:

• Aug 4-5 - All Day - Fall Choral Retreat (Apollo & Athena only)
• Sept 6 - 9am-3pm - Apollo/Athena Retreat
• Sept 18 - 7pm - NW Rutherford MS Mass Choir (Athena & Apollo only)
• Sept 27 - 9am-3pm - Athena/Apollo Retreat
• Dec 4 - Concert at RSMS (Meistersingers and Apollo only)
• Feb 7 - 9am-3pm - Apollo/Athena Retreat
• Mar 2 - Possible visit to SCMS Choir (Apollo only)
• Mar 10 - 3:30-5:30pm - Visit to SCMS (Apollo & Athena only)
• Mar 10 or 17 - 6pm - Freshmen Orientation (Meistersingers & Apollo only)

Additional, non-required activites (such as honor choir auditions and honor choir events), are on the Online Choir Calendar.  If you think you'd like to participate in these (a mmajority of choir students choose to do so), please make notes of these dates now.

Specific Call Times, along with all other information about a choir event, are found on gig sheets and on the Online Choir Calendar.  The Stewarts Creek Choir Calendar can always be found at www.StewartsCreekChoirs.com or on the Stewarts Creek High School website for reference throughout the year.  Please make yourself aware now of all choir events found on the Online Choir Calendar.  Put these events on your calendar now and please do not schedule any other events on these dates
