In order to begin services we will need copies of the following documents:
1. An ABA referral from an MD or Psychologist.
2. Copies of any diagnostic paperwork related to the diagnoses your child has.
3. Copies of your insurance card (front and back). Those documents can be uploaded here. If you do not have them at this time, they can be faxed to us at 833-550-1727 or emailed to info@SoutheastABA.com
By signing below you confirm that the above information is correct to the best of your knowledge, and that: It is your responsibility to inform us of any changes to your insurance coverage. Failure to do so may result in you being directly billed for all charges if your claims are denied. If your insurance policy requires a prescription from your PCP, it is your responsibility to provide that to Southeast Behavioral Services prior Not all services are a covered benefit by all insurance plans It is your to your first appointment responsibility to be aware of what services are being provided to you and if they are a covered benefit under your insurance policyYou are responsible for any charges for services not payable by your insurance policyYou authorize your insurance benefits to be paid directly to your provider You authorize Southeast Behavioral Services and your insurance company to release any information required to process claims We recognize that temporary financial problems may affect payment on your account. Please contact us promptly for payment arrangements if you are unable to pay in full.
