Hi there! If you're interested in booking an appointment with me, Tacho, please fill out this form and I will get back to you as soon as possible.

Booking Process:

1. You submit this booking form

2. I email you and we discuss any questions/details (dates, quotes etc)

3. We agree on the date and you send a $100-$150 non-refundable deposit/booking fee for the appointment. The booking fee will go towards the total cost of the tattoo. Please read the deposit/booking fee policy I've included later in this form.

4. I receive the deposit/booking fee and send you an email to confirm the appointment with a google calendar invitation. I'll see you then! Feel free to read up on my FAQ page before the appointment.

Custom Work:
Please try to be as detailed as possible when describing your idea. You should know what you are looking to get and I would be the one to help execute it. I do not show designs prior to the appointment as discussing adjustments are better in person during the beginning of the appointment. If there's anything specific you'd like to incorporate, please make a note of it.

Booking Fee/Deposit Policy
Terms used interchangeably.

This policy applies to out-of-town/traveling clients as well.

Deposits/booking fees are valid for one (1) rescheduled date if done within 48 hours prior to the appointment time and date. No questions asked and you don’t need to tell me the reason!

The booking fee will go towards and be subtracted from the total cost of the tattoo and you will only need to pay the estimated difference at your appointment. Booking fees are still non-refundable.

If you are a no-show or cancel last minute with a deposit/booking fee, you will forfeit your booking fee.

If you are 15+mins late without letting me know, your appointment is canceled and the booking fee is forfeited. If your GPS says you’re running late, you can email me before you start driving so you don’t have to worry about it! (I may not email back for safety reasons)
Quotes and date will be provided in my initial email response. Please triple check that it is the correct email address! I will get back to you very soon. Please remember to check your spam/promotional folder as well.
