Saturday, August 1, 2026 – Boys Teams
Location: Anderson High School Soccer Complex

Event Format
11v11 matches, multi levels of play
Each team will play 3 40-minute matches against different schools
Cost
$5 per player fee to help cover the cost of officials (can be paid the day of the event)
$5 Single Admission or $15 presale-only family pass for spectators

We plan on offering differnt levels of play this Summer. In a perfect situcation we would have

1. High Varsity 2. Mid Varsity/High JV 3. Low Varisty/JV/C

Informed Consent and Acknowledgement

I hereby give my approval for my Team’s participation in any and all activities prepared by the AHS Boys Soccer Program during the selected Event. In exchange for the acceptance of said Team's Participation in this eventby the AHS Boys Soccer Program and staff, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Anderson Community Schools, AHS Boys Soccer Program and all its respective officers, agents, and representatives from any and all liability for injuries to said player on this team arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said Player, I hereby waive all claims against Anderson Community Schools, AHS Boys Soccer, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

I also consent to allow my team/players to be photographed for social media and camp/program promotion. I understand I nor my team or players will not be compensated.

Confirmation

BY AGREEING ABOVE AND SUBMITTING THIS FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT IS EQUIVALENT TO AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
