Consent and Liability Waiver
In consideration of Euro Elite Hockey’s programs allowing me (or my child/ward) to participate, I acknowledge and agree to the following:
Program Adjustments: Euro Elite Hockey reserves the right to reschedule, modify, or regroup players based on enrollment, availability, and program needs.
Release of Liability:
I understand that participating in hockey-related activities involves inherent risks, including, but not limited to, physical injury, illness, equipment damage, or property loss.
I agree to release, discharge fully, and hold harmless Euro Elite Hockey, the hockey complex, its directors, coaches, staff, and affiliates from all claims, liabilities, demands, and expenses arising from participation, regardless of cause.
Medical Acknowledgment & Responsibility: I verify that I, my child, or my ward have no medical conditions that would prevent safe participation in this program and that we are in good physical health.
I understand that Euro Elite Hockey, the hockey complex, and associated personnel will not be responsible for any medical, dental, or insurance claims, including those resulting from injury or illness during participation.
In case of emergency, I authorize Euro Elite Hockey staff to provide basic first aid or seek medical assistance, with the participant or guardian bearing all costs.
Assumption of Risk: I recognize that hockey is a high-intensity sport involving body contact, fast movement, and potential hazards. By signing this waiver, I voluntarily assume all risks associated with participation and accept full responsibility for any injury or damages that may occur.
Media Release & Social Media Consent:
I grant Euro Elite Hockey permission to photograph and video record me (or my child/ward) during program activities.
I understand these images and videos may be used for promotional materials, social media posts, websites, and other public platforms related to Euro Elite Hockey.
I waive any rights to compensation or claims regarding using these images or videos.
Acknowledgment of No Refund Policy: I acknowledge that, once the program has begun, no refunds or credits will be issued, regardless of the reason for withdrawal or missed sessions.
By signing below, I confirm that I have read, understood, and agreed to the abovementioned terms.
Cancellation Policy
At Euro Elite Hockey, our camps and programs are planned based on confirmed enrollment numbers well in advance to ensure a high-quality training experience for all participants. Due to this, we do not offer refunds or cancellations, except in cases of medical necessity.
Refund & Credit Policy:
Suppose a player cannot attend due to medical reasons and provides notification before the program starts. In that case, they will receive a credit for the full amount paid, minus a $50 administration fee. This credit can be applied to any Euro Elite Hockey camp or program within the following calendar year.
A doctor’s note is required to qualify for medical credit and must be submitted within one week of notification.
No Refunds After Program Start: Once the program has begun, no refunds or credits will be issued for missed sessions or withdrawals, regardless of the reason.
Please note that credits are non-transferable and cannot be shared or used by another participant. We appreciate your understanding and support, as this policy helps us maintain the highest level of organization and commitment to all players.
