EURO ELITE HOCKEY
Participant Waiver, Consent & Program Policies
Applicable to camps, programs, and private training sessions
Please read carefully before signing.
1. Acknowledgment & Agreement
In consideration of Euro Elite Hockey allowing me, or my child/ward (the “Participant”), to take part in its camps, programs, and/or private training sessions (collectively, the “Programs”), I acknowledge and agree to the following:
2. Program Structure & Adjustments
Euro Elite Hockey reserves the right to:
Modify schedules, session formats, or groupings
Reassign participants based on skill level or enrollment
Adjust sessions due to facility availability or operational needs
Provide reasonable notice whenever possible.
3. Private Session Scheduling Policy
For private training sessions:
A minimum of 24 hours’ notice is required to reschedule
Late cancellations (<24 hours) may result in loss of the session without refund
No-shows will be charged in full
Sessions are subject to coach and facility availability
Euro Elite Hockey may reschedule due to unforeseen circumstances
4. Assumption of Risk
I understand that participation in hockey-related activities involves inherent risks, including but not limited to:
Physical injury (including serious injury or death)
Collisions, falls, and contact with players, boards, or equipment
Equipment failure
Illness or infection
Property damage or loss
I voluntarily assume all risks associated with participation.
5. Release of Liability
To the fullest extent permitted by law, I release and hold harmless Euro Elite Hockey, its directors, coaches, staff, volunteers, affiliates, and associated facilities (the “Released Parties”) from any and all claims, liabilities, damages, or expenses arising from participation in the Programs.
This includes claims arising from negligence but does not apply to gross negligence or willful misconduct.
6. Indemnification
I agree to indemnify and hold harmless the Released Parties from any claims, damages, or costs (including legal fees) arising from the Participant’s conduct or participation.
7. Medical Acknowledgment & Responsibility
I confirm that the Participant is physically fit to participate in high-intensity hockey training and that any medical conditions, injuries, or limitations have been disclosed prior to participation.
I understand that Euro Elite Hockey does not provide medical or insurance coverage and is not responsible for any medical, dental, or related expenses.
8. Emergency Medical Authorization
In the event of an emergency, I authorize Euro Elite Hockey staff to:
Provide basic first aid
Contact emergency services (911)
Arrange transport to a medical facility
Consent to necessary emergency treatment if I cannot be reached
All associated costs are the responsibility of the Participant or guardian.
9. Media Release
Euro Elite Hockey may photograph or record participants for promotional purposes, including use on websites, social media, and marketing materials.
Consent is optional and does not affect participation eligibility.
Cancellation & Refund Policy
Euro Elite Hockey schedules programs in advance based on confirmed registrations.
Camps & Group Programs
No refunds or credits will be issued after the program start date
Missed sessions are non-refundable
Medical Credit (Pre-Program Only)
If a Participant cannot attend due to medical reasons before the program start date:
Credit issued minus $50 admin fee
Doctor’s note required within 7 days
Credit valid for 1 year
Credit is non-transferable
No credits will be issued after the program begins.
Private Sessions
All private sessions are non-refundable
Rescheduling allowed with 24+ hours’ notice
Late cancellations or no-shows result in loss of the session
10. Payment Policy
Payment is required in advance unless otherwise agreed.
Credits (if issued) must be used within the stated validity period.
Unused sessions may expire at the discretion of Euro Elite Hockey.
11. Governing Law
This agreement shall be governed by the laws of the Province of British Columbia, Canada. Any disputes shall be resolved exclusively in the courts of British Columbia.
12. Severability
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
13. Acknowledgment & Signature
By signing below, I confirm that:
I have read and understood this agreement
I understand that this is a legally binding waiver of rights
I agree to all terms, including the Liability Waiver and Policies
