By submitting this form, I am officially registering for Kids Soccer with Community First Sports Club (CFS) and am agreeing to all of the following items to be known as the ("Terms and Conditions"):

 

• Kids Soccer is a single season commitment to attend one, two, or three classes per week for the entire season I have registered for. I understand that there are no refunds even in the case that my player has not been regularly attending and that failure to attend classes may result in my players removal from the program.
  

 

• I am reserving a place for my player in the CFS Kids Soccer. There is no requirement for CFS to offer a refund if you withdraw your player from the team after completing payments. Credit towards future programs will be offered in the case of withdrawing your student before the start of the season.
  

 

• I have read, reviewed with my family, and accept CFS "Assumption of Risk and Liability Waiver with Regards to Coronavirus/COVID-19"

 

• CFS reserves the right to take video and photography of all players in practice and game setting to utilize for online media platforms, and for advertising, marketing, and other promotional material.
