Acknowledgment of Risks
I understand that yoga involves physical movement, stretching, bending, and other postures, which may carry inherent risks, including but not limited to physical injury. I acknowledge that I am voluntarily participating in yoga classes at Homestead Yoga and assume full responsibility for any risks, injuries, or damages, known or unknown, that I might incur as a result.
Health and Medical Conditions
I affirm that I am in good health and capable of participating in yoga classes. I agree to inform the instructor of any medical conditions, injuries, or limitations that may affect my practice. I understand that yoga is not a substitute for medical attention, diagnosis, or treatment.
Additional Acknowledgment for Goat Yoga Participants
I understand that goat yoga sessions include interaction with live animals, whose behavior may be spontaneous or unpredictable. I recognize the inherent risks involved, such as accidental scratches, bites, or unexpected contact, and I accept these as part of the unique experience. To ensure the safety of all participants and animals, I agree to follow all instructions provided by the Homestead Yoga staff and
exercise appropriate caution. I voluntarily assume full responsibility for any risks or injuries that may result from my participation in goat yoga activities.
Release of Liability
In consideration of being permitted to participate in yoga classes at Homestead Yoga, including goat yoga sessions, I, on behalf of myself, my heirs, personal representatives, and assigns, hereby release and discharge Homestead Yoga, its instructors, employees, and agents from any and all claims or causes of action arising out of my participation.
Consent for Emergency Medical Treatment
In the event of an emergency, I consent to receive medical treatment deemed necessary by emergency personnel.
Agreement and Signature
By signing below, I acknowledge that I have read and understand this waiver and release form. I voluntarily agree to its terms. For participants under the age of 18, a parent or legal guardian’s signature is required.
