    This form can be used as either a Performance Improvement Plan or a Counseling Form.
    Please complete all sections in advance, save your work, and review it with the employee during your meeting.

    After reviewing, both you and the employee will sign, enter your email addresses to receive a copy, and submit the form. Once submitted, it will be sent to HR for filing in the employee’s record.
