Instructions for Filling Out the Sponsorship Letter Request Form
CSA Biñan Grand Alumni Homecoming 2025 and KISLAP 2025

This form is intended for alumni, batch representatives, committee members, and endorsed volunteers requesting an official sponsorship letter on behalf of the GAHC 2025.

Please follow the process below:
1. Fill out the form completely and accurately. Provide all required details, including sponsor information and purpose of the sponsorship. Indicate whether the letter is for personal or corporate use.
2. Submit the form. After submission, you will receive an acknowledgment email confirming receipt of your request.
3. Routing and review. Your request will be routed to the Ways and Means Committee and reviewed as soon as it is received.
4. Approval and letter generation. If approved, a control number will be assigned and a personalized sponsorship letter will be generated and sent to your email in PDF format.
5. If clarification or revision is needed, you will be notified via email. Tracking and confirmation.

Please ensure that the email addresses of both the requestor and the sponsor are valid and accurate. Incorrect entries may delay the processing or delivery of your sponsorship letter.
