Note:

• Do not submit a fee waiver request if you have already paid for your application.
  A refund will not be issued.
• Submit only ONE request for an initial that you will be using in the next three months.
• If you submit multiple initials waivers we will mark your waiver invalid and require you to resubmit a new form for a waiver.
• Waivers are NONTRANSFERRABLE, for example if you give your waiver to someone else, try to apply an initials waiver for a renewal or any combination of other circumstances we deem invalid, IT WILL BE REJECTED.
• If you submit an invalid combination of waivers, for example you request two waivers that can be applied for the same thing, such as an CADC II initals waiver and an InterCoast College waiver, your waiver will also be deemed invalid and rejected.

RULES

• DO NOT use this form if you need a waiver for renewals, your payment step will be REJECTED and your waiver will be invalidated.
• DO only use this form for credentials you will be applying for soon (waivers will NOT be valid in 3 months - end of August)
• DO NOT apply for multiple initials waivers (we will invalidate ALL of those waivers)
• DO NOT apply for and invalid combination, a combination that you can not use all together, for example an "Advancement with Exam" and an "Advancement (No Exam) Waiver"
• DO NOT apply for DUPLICATE WAIVERS, meaning do not apply for waivers that can be used for the same thing, we will know and invalidate both.
• DO NOT apply for a waiver if you have already paid. No refunds will be issued for ANY reason.

CCAPP reserve the right to invalidate any waivers.
