Tennis Contract Application
Directions for completing this application:
1. Make sure all fields are complete and all information is provided prior to submitting.
2. For solutions to questions or if you need assistance, please contact contracts@tricitytennis.com or schedule a player meeting with Maddy by using the following link https://calendly.com/maddy-tricitytennis/tricity-player-meeting
3. Contract Application deadline is June 30, 2025. All contract heads will be notified July 15th of the finalized contract day/time. All contract members will see contracts posted to their account by July 15 and payment will follow the above schedule (25% by August 1, 50% by Sept 1, 75% by Oct 1, 100% by Nov 1). Any contract that does not follow these guidelines will risk losing their contract.
Days and times will not be guaranteed to stay the same as the previous seasons but existing contracts will receive priority over new contracts, assuming the application is submitted before the deadline.

All Court Assignments are at the discretion of the club and may change over the course of the season.

Tri-City will notify the Contract Head when the contract days/times are finalized.

Contract Head Agreement

By signing this contract, the Contract Head and all players acknowledge and agree that there are certain inherent dangers in playing tennis and Tri-City Fitness, Inc. shall not be liable for any personal injuries, property damage or other loss sustained by the undersigned in, on or about the premises of the Club or arising out of the use or intended use of any facilities, equipment or other property of the Club, whether or not said personal injuries, property damage or other loss sustained by the undersigned is the result of the negligence of the owners, agents or employees of the Club or the negligence on any other persons present on the premises of the Club. These conditions apply individually and/or jointly with other players, players' children or guests of players.
