Integrated Diabetes Services (including its owner and staff), henceforth referred
to as IDS, does not prescribe medications. DIY Loop and other Open Source
Systems are not approved for therapy by any official government agency.
As a matter of sound and ethical care, IDS provides education and consultation in
order to enhance the safety and efficacy of any form of diabetes treatment chosen
by our clients. This includes offering specific guidance to optimize glycemic
control.
Should you choose to utilize a non-approved/non-regulated device or medication,
or use an approved device or medication for an unapproved purpose, it is with the
understanding that you do so at your own risk. You take full responsibility for
building and running this system and you do so at your own risk. You agree to
review/discuss any such uses with your prescribing physician prior to
implementation. You also agree to release, indemnify and hold harmless IDS from
any and all damages, costs or expenses (including attorney’s fees) which you may
incur in connection with the use of an unapproved medical treatment.
Before scheduling a visit to build or rebuild your Loop or Open Source
System, you need to have all of the required, compatible, physical and software components available, including usernames, passwords, and other codes as outlined in the Pre-Build Checklist provided by Integrated Diabetes Services as a tool for your preparation and use.
I agree with the terms stated above.
