2025 Vacation Bible School Registration Form

VBS runs from July 28th - August 1st, 8:45 am to Noon. VBS is geared toward students going into Kindergarten through 5th grade. Students going into 6th-12th grade are welcome as helpers. Please register all students, regardless of grade, so we have all of their emergency information on file. Registration fees help cover the cost of snacks, supplies and a t-shirt. Registration fees for grades K-5 are $20 per student. As 6th -12th grade students are volunteering their assistance, their fee is reduced to $10 per student. Limited bus service is available at $10 per student. Registration deadline is July 7th, 2025 to ensure we have enough supplies. You can still register through the first day, however we cannot guarantee that we will have a t-shirt available for your student if you do not register by July 7th.

Permission and Release
I/We hearby consent to my child/children participating in the Holy Trinity Faith Formation activities, those on-site and those off-site. I/We understand that some events may take place away from the parish grounds and that my/our child will be under adult supervision and all reasonable caution will be taken to prevent accidents or injuries. I/We here by release and indemnify Holy Trinity Parish, its staff and volunteers, and the Diocese of Cheyenne from any and all liability arising from claims of any kind or nature whatsoever from my/our child's/children's participation in this event, including transportation.

In the event my/our child/children were to become injured, I/we give permission for the supervising adults to seek medical care or treatment for my/our child/children. I/We understand that if my/our child/children does become sick or injured, every effort will be made to contact a parent or guardian. If Holy Trinity can not reach a parent/guardian, they will contact the emergency contact in their records.

Bus Service Request

Bus Service is available for those families with extenuating circumstances that are unable to provide transportation to and from VBS. There will be a fee of $10 per student. Service is only available within Cheyenne city limits.

Payment

Payment may be made online or by cash/check. No student is ever refused due to inability to pay. Please contact the office for assistance.

If paying by cash or check, please mail or drop off at the parish office. You may also place in an envelope clearly labeled "VBS" and place in the collection at Mass.

If you are not currently using online giving, you will need to create an account. If you do use this service, you may log in using your current account information.
