  • ADULT RELEASE WAIVER

  • Participant Note:

    This form is required for any adult (18+) who is participating in SEEAG programming, including educators, volunteers, and chaperones.

    The purpose of participating in activities organized by Students for Eco-Education & Agriculture (SEEAG) is to promote education and exploration of agriculture, nutrition, and science through our programs-Farm-to-Food Lab, STEM Career Pathways in Agriculture, Farm Day, and the Youth Wellness Initiative, as well as general volunteer opportunities at our farm sites. By signing this waiver, you acknowledge and accept the risks associated with these activities.

    SEEAG Activity Locations:

    1. Ventura County Farm-to-Food Lab - School premises & the following farm education site:

    a. Petty Ranch, 11971 Darling Road, Saticoy, CA 93004.

    2. Santa Barbara County Farm-to-Food Lab - School premises & the following farm education site:

    a. Allan Hancock College Vineyard, 800 College Dr, Santa Maria, CA 93454.

    i. Southwest end of campus, next to the baseball fields (Coordinates: 34.942301, -120.426712 

    3. S.T.E.M. Career Pathways in Ag - School premises & various field trip locations in Santa Barbara and Ventura County

    4. Youth Wellness Initiative - School premises & various pop-up locations in Santa Barbara and Ventura County

    5. General Volunteer Days - Petty Ranch, Allan Hancock College Vineyard, or other SEEAG-approved locations.

    a. Activities may include gardening, harvesting, site preparation, and general farm tasks.

     

    Completion Required Before Participation:

    I understand that completion of this waiver is required before my participation in any SEEAG activities.

    General Guidelines and Acknowledgments:

    • I understand that SEEAG programs may take place in various locations, including school premises and field trips. I will follow all safety guidelines and instructions provided by SEEAG staff and representatives.
    • I will wear appropriate attire suitable for the activities to ensure my safety during the programs: Close-toed shoes, layers, and sun protection as necessary.
    • I acknowledge that the programs may involve physical activities and possible exposure to outdoor elements, andI assume full responsibility for any associated risks.

    Health and Safety:

    • I agree that if I am feeling unwell or displaying symptoms of illness before the scheduled activities, I will refrain from participating to safeguard the health and well-being of all participants.
    • I will remain with a SEEAG Educator or authorized representative at all times during the activities and will promptly notify them in case of an emergency.

    Food Safety and Medical Information:

    • SEEAG programs may involve food-related activities. I will provide accurate and up-to-date information on my food allergies, dietary restrictions, and other medical conditions that SEEAG should be aware of:
    • I will ensure this information is communicated to the teaching staff prior to participation.
    • SEEAG staff will make reasonable efforts to accommodate allergies or restrictions during food-related activities.
    • While precautions will be taken, I understand that accidental exposure to allergens may occur.
    • I understand that food-related activities are part of SEEAG programs, and I will follow all food safety guidelines and instructions provided by SEEAG staff to minimize the risk of any food-related hazards.
    • I will adhere to proper hygiene practices before and after handling food and using restroom facilities.
  • Release and Indemnification:

    In consideration of my participation in the organized activities facilitated by SEEAG at various locations, I, on behalf of myself and my family, voluntarily release, waive, discharge, and hold harmless SEEAG, its officers, directors, employees, contractors, representatives, agents, successors, and assigns (the "Releasees") from any and all liability for any injuries, illnesses, damages, or losses that may arise from or in connection with my participation in the activities.

    I agree to indemnify and hold harmless the Releasees from any liability, demand, damage, claim, cost, loss, or expense (including attorneys' fees) arising from my participation in the activities, except for cases of gross negligence or intentional misconduct by the Releasees.

    I acknowledge that my participation in the activities is voluntary and that I assume full responsibility for any risks, injuries, illnesses, or damages that may occur. I understand that I am waiving certain legal rights and remedies, and I am aware of the inherent risks associated with the activities.

    I have read and understood this waiver, and I voluntarily agree to its terms. I acknowledge that this waiver is intended to be broad and inclusive, and if any part of it is found to be unenforceable, the remaining provisions shall remain in full force and effect.

    I acknowledge that this agreement is governed by the laws of the State of California.

    This waiver will remain valid for a period of three (3) years from the date of submission, unless revoked in writing. Participants are responsible for notifying SEEAG of any changes to their medical information, contact details, or participation status during this period.
