I, the undersigned, am the parent or legal guardian of the above-named child. I understand and acknowledge that participation in Kids Night Out at The Craft Witch involves supervised arts and crafts activities, food consumption, and film viewing.

I hereby release, waive, and hold harmless The Craft Witch, its employees, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or illness (including allergic reactions) that may be sustained by my child while participating in this program.

I give permission for my child to receive basic first aid if necessary and understand that I will be contacted immediately in the event of an emergency.

I also consent to the use of any photographs taken during the event for promotional purposes, unless I notify staff otherwise in writing.
