  • Some older people need extra, intensive support to access aged care services and other supports in the community.

    In these situations, a care finder may be able to help.

    Care Finder is a free service. It exists to support vulnerable people – who have no one else who can support them – to learn about, apply for and set up support services.

    If someone doesn’t meet the criteria there are a range of other support options available by visiting the My Aged Care website or COTA Tasmania Care Finder webpage for more information on the supports that are available to you and how to access them.

    Don’t worry if you are unsure if the Care Finder program is right for you. We will contact you, when we call, we will ask some questions so we can determine the best way to support you.

     

    To be eligible for Care Finder support you will need to meet the following criteria.

    At all times we try to only collect the information we need for the particular function or activity we are carrying out. The main way we collect personal information about you is when you give it to us, or for example, personal information such as contact details and support needs. Sometimes we need to collect sensitive information to help us connect you to the right supports. This might include information about your health, needs, circumstances or culture. We only disclose your sensitive information for the purposes for which you gave it to us. We will ask for your consent before we do so.
