  • Thank you for your interest in becoming a National Football Foundation, Southern Arizona Chapter (NFFSAC) 12th Man, in Tribute to Joel Favara, Nominee. The following application will require personal information input, short paragraph writing, nominee's unofficial transcript and athletic/academic resume* upload, and two (2) Letters of Recommendation. The Letters of recommendation may come from the student-athlete's Head Coach, Position Coach, teacher, school administrator, communicty leader or employer. The Nominee must be a senior, student-athlete who is part of the 2025 high school flag football or tackle football program; as a player or support personnel. Additionally, the following guidelines for acceptance of a nomination shall apply:

    1. Student-athlete must show a minimum 3.00 cumulative grade point average (GPA) based on a 4.0 scale.

    2. An unofficial & unlocked copy of the student-athlete's transcripts through November 2025 

    3. A complete application with all materials requested** by SUNDAY, December 14, 2025

    * An Athletic/Academic Resume is requested, not required

    **Letters of Recommendation may be uploaded with this application or submitted electronically to nffsoaz@gmail.com no later than Thursday, December 18, 2025.

    WE WILL ONLY RECEIVE AND REVIEW APPLICATIONS SUBMITTED ELECTRONICALLY VIA THIS PORTAL.
