This form is required to sign your child up as a model. Please complete by June 27th to make sure your child is on our model list.

Special Note: By signing up for this event, you understand that you and
the members of your party, may be photographed or videotaped, and you
authorize Kids Center for Pediatric Therapies the perpetual right to use
your likeness or image without compensation, for use in any medium, for
marketing and fundraising purposes.

WHAT IS IN THIS PACKET?

Parent Instructions	Includes key dates, deadlines, locations, directions, etc.
Model Questionnaire	Must be filled out completely by the deadline.
Thank You Note	Thank you notes are available at the front desk of both Kids Center campuses, can be emailed upon request, and will be available at the Fashion Show. The thank you notes will be for our event sponsors, volunteers, and donors.
Upload a Photo	There will be a slideshow playing during the event. If you would like to include a photo of your child, scroll to the bottom to upload. *This is not mandatory

IMPORTANT DATES

June 27th	5:00 pm	Deadline to enter Fashion Show- must have all forms turned in at Kids Center, or completed/submitted online
July 24th	10:00am-4:00pm	Kohl's Shopping Day
August 2nd	10:30-Noon	Rehearsal at The Jeffersonian 10617 Taylorsville Rd.
August 3rd	1:00pm	Arrival for lining up. Your time will be determined by your group. Information to follow.
August 3rd	2:30pm-4:30pm
