Boarding Owner's Release Form

I understand that you cannot guarantee the health of my pet(s). I understand and will not hold Woodmoor Veterinary Hospital & Pet Lodge responsible for conditions that are unavoidable in boarding kennels such as but not limited to: weight loss, hair loss, upper respiratory infections, bronchitis, diarrhea and fleas. I understand that all pets admitted to the hospital must be protected against communicable contagious diseases and must be free of internal and external parasites or will be treated upon entry or discovery at the owners/ agents expense.
I understand that in the event of an illness, the staff will immediately attempt to contact me or my agent to discuss the problems and treatment options, but may not be able to reach me immediately and is therefore authorized to initiate appropriate treatment until myself or my agent can be reached.

Should an EMERGENCY arise after hours, I authorize my local emergency contact to pick up my pet and transport them to the closest Veterinary Emergency Hospital:

I understand that Woodmoor Veterinary Hospital & Pet Lodge is not responsible for loss or damage to personal items left with the pet(s) including but not limited to leashes, collars, toys, and bedding.
The hospital is to use all reasonable precaution against injury, escape, or death of my pet. The hospital and its staff will not be held liable for any problems that develop provided reasonable care and precautions are followed. I understand that any problem that develops with my pet will be treated as noted above and I assume full responsibility for the expenses incurred.
I will call if my “pick up date” changes so that you can plan accordingly. If I neglect to pick up my pet(s) within 10 days of the date scheduled for discharge, and do not notify you within that time period, you may assume that my pet(s) are abandoned and is hereby authorized to take responsibility for my pet(s) including adoption and/or other options considered necessary (including euthanasia).
I have been provided with a copy of the boarding policy explaining the boarding policy and regulations of Woodmoor Veterinary Hospital & Pet Lodge.
I understand there is an additional charge for any pet deemed aggressive during the boarding stay.
