Requirements for boarding:
· All dogs:
o Must be healthy and have current immunizations against Rabies, DAPP, Bordetella, and Leptospirosis.
o They must also have a current Exam with us or your primary veterinarian and be free of parasites (fleas, ticks, tapeworms, etc.)
· All cats:
o Must be healthy and have current immunizations against Rabies and FVRCP.
o They must also have a current Exam with us or your primary veterinarian and be free of parasites (fleas, ticks, tapeworms, etc.)
Terms and Conditions:
· All clients are required to read and sign the Boarding Admission Form and the Owner Release.
o A reservation is not guaranteed without these documents completed and must be verbally confirmed with one of our Pet Lodge team members.
· Animals will be admitted and discharged ONLY during the stated times below:
o Monday – Friday 7:30am to 4:00pm.
o Mon. – Fri.: We are closed for drop offs and pickups from 12:00pm to 1:00pm.
o Saturday – Sunday 8:00am to 9:30am and 3:30pm to 5:00pm
o Pets taking medications MUST be dropped off Monday-Friday.
· Pets dropped off or picked up after 4:00pm will be charged a $25 late drop off/ pickup fee per pet.
· Pets picked up or dropped off during the lunch hour (12pm-1:00pm) during the weekdays will be charged a fee of $25 per pet.
· Any animal that requires emergency or immediate veterinary attention will receive it at our discretion during business hours. If a pet has a life-threatening medical emergency your pet will need to be transported to a 24/7 Veterinary Emergency Center by the local emergency contact provided.
· After hours care is not available at Woodmoor Veterinary Hospital.
· A local emergency contact must be listed and available to pick up your pet in the event of an emergency or if we deem the pet no longer fit for boarding due to medical or behavioral concerns for the safety of all animals staying in our Pet Lodge.
o They will also need to be authorized to make medical decisions on your behalf if we are unable to contact you.
· All Canine overnight reservations include two outdoor playtimes (we cannot accommodate group play unless pets are within the same household), up to three feeding times per day, raised cots, blankets, and lots of love!
· All medications must be in the original packaging with the instruction label provided.
· Animals requiring medication or treatment will be charged appropriate fees.
· Woodmoor Veterinary Hospital & Pet Lodge cannot guarantee the health of your pet(s) and will not be held responsible for conditions that are sometimes unavoidable in boarding kennel, such as but not limited to: weight loss, upper respiratory infections, self-harm or diarrhea.
· For the health and wellness of the other pets in our facility, we may not accept pets with open wounds or sores.
· Rawhide bones, rope toys, bowls/ dishes are not permitted in our boarding facility. Woodmoor Veterinary Hospital & Pet Lodge will not be responsible for any personal supplies, toys, blankets, etc. left with any pet(s).
· I acknowledge that my pet may be given human-grade additives (pumpkin, peanut butter, etc.) to their meals, medication administration, or as a treat, unless specified otherwise.
· If I neglect to pick up my pet(s) within 10 days of the checkout date provided, Woodmoor Veterinary Hospital may assume my pet(s) has been abandoned and is hereby authorized to take responsibility for my pet(s) including adoption and/or other options considered necessary (including euthanasia).
· I hereby authorize Woodmoor Veterinary Hospital & Pet Lodge, to post and display pictures and/ or video of my pet(s) on their website, social media sites, or promotional materials. This photograph/ video will not be used for any other purpose without my express written/ electronic permission.
· I understand that Woodmoor Veterinary Hospital does not provide 24-hour supervision.
· I understand that any in-house food that is given to my pet incurs a $2 meal charge.
· I understand that medications are priced via tiered levels as follows:
o Supplements - $2 per day.
o Medications Level 1 - $4.50 per day. Covers 1-4 oral medications, or eye medications.
o Medications Level 2 - $6.50 per day. Covers 4+ oral medications, eye medications, or any ear medications.
o Medications Level 3 - $8.50 per day. Covers medications that require any extra time for administration such as pets that need to be pilled, ear flushes, inhalers, etc.
o Injectable medications - $10.00 per day. Covers injectable medications including insulin, b12, etc.
· I understand that holiday peak pricing includes an additional $10 per day during peak days and that during holidays I am required to pay a 50% deposit at the time of booking. Those days are as follows:
Spring Break: 3/17-4/1
Memorial Day: 5/23-5/27
Independence Day: 7/3-7/7
Labor Day: 8/29-9/2
Thanksgiving: 11/24-12/1
Christmas/New Year’s: 12/19-1/2
· Please make your boarding reservations as early as possible, especially during holidays and other busy travel times. Holiday boarding requires a 50% deposit and a 14-day cancellation notice in order to receive a full refund.
· If your pet develops diarrhea while staying with us, we will administer a probiotic at your expense.
· Overnight charges accrue starting the day of drop off regardless of the time your pet(s) arrive(s).

We would like to remind you that you are responsible for any charges incurred during your pet’s stay with us. Payment is due at time of service in the form of Cash, Check, Credit/ Debit, or Care Credit. If your pet’s stay exceeds two weeks, we will need a 50% deposit prior to his/ her/ their stay. If his/ her/ their stay is extended beyond prior agreed departure (exceeding two weeks), we will need a full payment of his/ her/ their current boarding and any medical charges upon this extension.
I agree to pay reasonable attorney fees incurred by Woodmoor Veterinary Hospital & Pet Lodge in the collection of any and all fees resulting from the care and maintenance of my pet(s). I further agree that in the case of non-payment, a finance charge of 1.5% per month (18% annually) will be charged.
It is understood and agreed between the owner and Woodmoor Veterinary Hospital & Pet Lodge, that this agreement contemplates that the owner will be in need of Woodmoor Veterinary Hospital & Pet Lodge’s boarding services from time to time in the future, and it is further understood and agreed that the obligation and convents herein shall apply to each provision of service in the future, without the need of entering into another agreement, unless altered in writing and signed by the parties herein.
I have read and fully understand and agree to the above terms. I understand and agree that all fees are due and payable when I pick up my pet(s). This policy and agreement shall apply indefinitely and during the course of each and every visit, I or my pet makes to Woodmoor Veterinary Hospital & Pet Lodge.
