NFFSAC Scholar-Athlete Honoree Application 2026

Thank you for your interest in becoming a National Football Foundation, Southern Arizona Chapter (NFFSAC) Scholar-Athlete Honoree. The application will require personal information input, scholar-athlete's unofficial transcript (through the sixth semester, unlocked document), scholar-athlete's athletic/academic resume (PDF format) and two Letters of Recommendation (PDF Format). The Letters of Recommendation may come from the scholar-athlete's Head Coach, Position Coach, teacher, school administrator, communicty leader or employer. The Nominee must be a senior, scholar-athlete who is part of the 2025 high school tackle or flag football team. Additionally, the following guidelines for acceptance of a nomination shall apply:

1. Scholar-athlete must show a minimum 3.00 cumulative grade point average (GPA) based on a 4.0 scale.

2. An unofficial copy of the scholar-athlete's transcripts through November 2025 (sixth semester)

3. A complete application with all materials requested* by Sunday, December 14, 2025

*Letters of Recommendation may be uploaded with this application or submitted electronically to nffsoaz@gmail.com no later than Thursday, December 18, 2025.
