WAIVER & AGREEMENT

Waiver & Agreement: In consideration of the athlete listed, hereinafter referred to as “Participant”, being allowed access to the facilities and/or programs of the Lewis County (LC) Youth Sports and Lewis County Adventist, Morton, Mossyrock, Boisfort, Adna, Pe Ell, Toledo, Winlock, Napavine, White Pass, Onalaska, Centralia, Chehalis, Rochester, and Elma school districts:

IT IS HEREBY UNDERSTOOD AND AGREED: I understand that this activity involves physical activity exertion and some physical contact and that participation in this activity could potentially result in serious injury arising us of conditions of the facility utilized, player conduct, equipment utilized, and other factors, including random chance. The LC Youth Sports and the above listed school districts, their owners, operators, employees and agents are hereby released from any and all claims for activity in connection therewith, whether such injuries or damages are foreseen or unforeseen. It is further understood and agreed that the undersigned agreed that the undersigned agrees to defend, hold harmless and indemnity the LC Youth Sports and the above listed school districts from any claim by the participant the undersigned, the participant’s family, estate, heirs or assigns.

Games will likely be on WEEKENDS, but if there is an excessive number of teams, ALL teams will be required/expected to play on WEEKDAYS.

I understand that there will be NO REFUNDS.

I further state that I am of lawful age and legally competent to sign this affirmation, release, hold harmless and indemnity agreement; that I understand the terms herein are contractual and are not merely recital; and that I have signed this document after reading it and as my own free act.

You recognize and agree that you shall not be entitled to a refund for any purchase under any circumstances. No Warranties LC YOUTH SPORTS MAKES NO WARRANTIES REGARDING THE PERFORMANCE OR OPERATION OF THIS WEBSITE. LC YOUTH SPORTS FURTHER MAKES NO REPRESENTATIONS OR WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, AS TO THE INFORMATION, CONTENTS, MATERIALS, DOCUMENTS, PROGRAMS, PRODUCTS, BOOKS, OR SERVICES INCLUDED ON OR THROUGH THIS WEBSITE. CONCUSSION INFORMATION

LC YOUTH SPORTS CONCUSSION INFORMATION:

As a parent and as an athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury. This form must be completed for every sports season and every youth athletic organization the athlete is involved with.

I have read the Parent Concussion and Head Injury information on lcyouthsports.com website and understand what a concussion is and how it may be caused. I also understand the common signs, symptoms, and behaviors. I agree that my child must be removed from practice/play if a concussion is suspected.

I understand that it is my responsibility to seek medical treatment if a suspected concussion is reported to me.

I understand that my child cannot return to practice/play until providing written clearance from an appropriate health care provider to his/her coach.

I understand the possible consequences of my child returning to practice/play too soon.

CODE OF CONDUCT

Parents will understand that LC Youth Sports activities are for the benefit of children and parents are required to conduct themselves in a manner consistent with creating a positive experience for their children, as well as other children and their families.

As a Parent or Guardian of a child registered with LC Youth Sports, you are responsible for the conduct of yourself, your child, your guests, and other family members. LC Youth Sports will not tolerate a parent yelling at or belittling a child, officials, coaches, or other parents in any way. All violations of the Code of Conduct will result in sanctions and may lead to the immediate expulsion of that parent from an event and other LC Youth Sports activities.

As an adult and role model, children are influenced by my words and actions. I agree to the following:

1. I will remember that all children participate to have fun and that the games are for youths, not adults.

2. I will be a positive role model for children and will encourage them to reach the highest levels of sportsmanship and to play by the rules.

3. I will be respectful and courteous to others, and will encourage players, umpires, officials and coaches through my positive actions and attitude.

4. I will not engage in any kind of unsportsmanlike behavior with an official, umpire, coach, player, or parent such as arguing, booing, taunting, or using profane language or gestures.

5. I will teach my child to treat other players, coaches, and officials with respect regardless of age, race, creed, color, gender, or ability.

6. I will not be aggressive or hostile towards another individual and will teach my child to resolve conflicts without resorting to violence or argument.

7. I will not ridicule or yell at my child or any other participant for making a mistake and will not hold a specific child responsible for losing a competition.

8. I will respect and support the officials and coaches’ authority during games and will never argue with, question, discuss or confront officials or coaches.

9. I will do my best to make youth sports fun for all children.

10. I will support LC Youth Sports, Coaches and parent volunteers.

11. I agree to pay LC Youth Sports all necessary registration fees. 

12. I agree to make efforts to keep facilities and the gyms we use clean.

13. We fully understand that the Coach of each team makes all decisions concerning practices, games, playing time and positions and Parents, Players, and all Spectators must respect these decisions.

After reading the PARENT-PLAYER CONTRACT, we, the legal Parent/Guardian, agree for our child to participate in LC Youth Sports League.

WE AGREE TO ABIDE BY THIS CONTRACT FOR ONE CALENDAR YEAR: June 2nd, 2025 - June 2nd, 2026
