This form must be completed by any adult planning to participate in Cotillion, including all volunteers and any parents planning to attend the Parent/Adult and Child dance.
I, {parent157}, hereafter referred to as “undersigned” or “parent volunteer” or “adult volunteer,” understand that participation in Ridgecrest Cotillion as a parent volunteer/participant is voluntary and not part of the regular school program. It is further understood that participation in Cotillion, by its very nature, may pose a high degree of risk of injury which may result not only from the actions, inactions, or negligence of Ridgecrest Cotillion, its affiliates, its instructors, any agents, employees and volunteers, but also the action, inaction, or negligence of others, the rules of participation, or the condition of the premises. The undersigned authorize Ridgecrest Cotillion, its affiliates, its instructors, any agents, employees and volunteers to obtain emergency medical treatment for Participant at the undersigned’s sole expense. Any negligence in seeking, or failing to seek, emergency medical treatment for Participant is included within the scope of this Release.
In consideration of being allowed to participate in any way in Ridgecrest Cotillion related events and activities, the undersigned:
1. Agree that the adult volunteer has the right to inspect the facilities and equipment to be used, and if it is believed that anything is unsafe, he/she should immediately advise supervisor of such condition(s) and refuse to participate.
2. Acknowledge and fully understand that each participating adult volunteer will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
4. Release, waive, discharge, hold harmless and covenant not to sue Ridgecrest Cotillion, its affiliates, its instructors, any agents, employees and volunteers, sponsoring agencies and owners and lessors of premises used to conduct the event from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the persons involved.
5. Shall indemnify and hold Ridgecrest Cotillion, its affiliates, its instructors, any agents, employees and volunteers from and against all allegations, claims, actions, suits, demands, damages, liabilities, obligations, losses, settlements, judgments, costs and expenses (including without limitation attorneys’ fees and costs) which arise out of, relate to or result from any act or omission of Ridgecrest Cotillion, its affiliates, its instructors, any agents, employees and volunteers.
The adult volunteer participant willingly and knowingly wishes to participate in Ridgecrest Cotillion in spite of the risks.
I HAVE READ THE ABOVE WAIVER AND RELEASE OF LIABILITY. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND AM SIGNING IT VOLUNTARILY.
Any adult wishing to participate in Cotillion as a volunteer or a participant in the Parent/Adult and Child dance MUST sign this form. A separate form must be completed for each adult participant.
