  • ASSUMPTION OF RISK, RELEASE & WAIVER

    This document affects your legal rights. Please be sure you understand it before signing and consult a personal attorney if you need assistance in doing so. Please sign and submit this series of release forms by July 1, 2025.
  • Activities for this program may include: 1) walking across campus between events; 2) voluntarily engaging in optional athletic activities during free time periods, such as exercise at the Fowler Center, basketball, kickball, frisbee-throwing, swimming in the University pool, etc.; 3) engaging in planned camp activities such as “SUMMA’s Got Talent” and an outdoor scavenger hunt; and 4) engaging in a traditional camp hike (approximately 1.5 miles each way and on flat ground).

    Note: Golf carts and vans are available to all campers with mobility issues or injuries, or who have requested a visitor accomodation through the University's ADA Office. 

    Code of Conduct: The University of the South has certain standards of conduct that apply to all students. As guests of the University, participants are expected to follow the University standards of conduct as well as rules that pertain specifically to participants in SUMMA. Camp directors and staff are empowered to enforce the standards of expected behavior and the Program Director has the authority to send a camper home for flagrant or repeated violations of these standards.

    The 2025 SUMMA Camp Handbook, including the Code of Conduct, can be accessed in here or on the SUMMA website FAQs page here. By signing this form, you confirm that you have reviewed the Camp Handbook in its entirety. You understand that these rules are in force throughout SUMMA Camp and that participation in the program may be terminated for violations of the Code of Conduct or any rules and regulations found throughout the handbook.

    Assumption of Risk: I wish to voluntarily participate in the Activity described above. I understand that participation carries certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. I understand that hiking involves walking on uneven terrain on and off primitive trails, over/under obstacles, with inherent risks of falling, tripping, etc. I understand that the University of the South does not guarantee my personal health and safety or protect me against risk of loss of personal property. Some of the risks of the Activity may include but are not limited to: transportation accidents, weather hazards and natural disasters, infectious diseases, slips and falls, burns, pinches, scrapes, scratches, sprains, fractures, and concussions. I also understand that injury and loss to me may result from unknown or unexpected risks and may result from the use of equipment, materials, or facilities provided by the University; environmental conditions; acts or omissions of others; and unavailability of immediate or adequate medical care. It is also possible that my participation in the Activity could result in injury or harm to a person other than myself, and I accept sole responsibility for my actions. I will not participate in any portion of the Activity if a medical condition prevents me from complying with these safety guidelines, even with an accommodation. I will also not induce others to participate in any portion of the Activity that might bring harm to them.

    Release and Waiver Claims: In consideration for this opportunity to participate in the Activity, I hereby release, discharge, and hold harmless the Board of Trustees of the University of the South and its officers, employees, agents and volunteers (collectively “University”) from all liability, and waive all claims, actions, demands, losses, damages, liabilities, and expenses (including attorney fees), for personal injury and bodily injury (including death) and for property loss and damage arising out of my participation in the Activity. If I am injured or become ill while participating in the Activity, I understand and agree that I am responsible for my own medical bills, including copayments and deductibles, and that I will not seek reimbursement from the University.

    Acknowledgement of Understanding: I have read and understand this Assumption of Risks, Release and Waiver agreement. I understand that by signing this agreement, I am giving up substantial rights, including my right to sue. By choosing to participate in the Activity and signing this agreement, I represent that I fully understand and assume the associated risks on behalf of myself as a participant or on behalf of the minor participant. I consent to medical treatment in the event of injury or illness during the Activity.

    I am signing this agreement freely and voluntarily and intend for this agreement to be a complete and unconditional release of all liability and claims as described to the greatest extent allowed by law. This agreement is binding on my heirs, personal representatives, assigns and those of the minor.

  • TRANSPORTATION PERMIT

    To be used throughout the duration of SUMMA on and off the University of the South campus, including transportation to/from the Nashville Airport, medical treatment, emergency medical transportation, and other activities as deemed necessary by SUMMA administration.
  • I hereby grant permission for the participant named above to travel during the duration of the Activity dates by: University owned vehicle, Commercial chartered or rented vehicle, and Emergency Medical Service vehicles.

    I understand the above-indicated transportation may be provided by the University or an independent contractor. To the fullest extent permitted by law, I waive, release, and discharge the University of the South, its employees, Boards of Regents and Trustees, and agents from any claim, demand or cause of action arising out of or related to the transportation provided and agree to indemnify and save harmless the University, its employees, Boards of Regents and Trustees, and agent from all claims for loss, damage, or injury sustain by me or my child whether the same be cause by the negligence of the University or otherwise. I agree that the transmission of this signed release by facsimile or other electronic transmission is sufficient to replace an original signed release.

  • MEDIA CONSENT & RELEASE AGREEMENT

  • I authorize the University of the South (University), and those acting pursuant to its authority, a non-exclusive, perpetual, worlwide license to:

    • Record my participation, likeness and/or voice on a video, audio, photographic, digital, electronic, hosted media, web-based service or any other medium, including podcasting.
    • Use my recorded likeness and/or voice on a video, audio, photographic, digital, electronic, hosted media, web-based service or any other medium, including podcasting.
    • Use my name and identity in connection with these recordings. 
    • Use, reproduce, exhibit, or distribute in any medium (e.g., print publications, video tapes, CD-ROM, Internet, podcast) my recorded likeness and/or voice on a video, audio, photographic, digital, electronic, or any other medium without restrictions or limitations for any educational or promotional purpose which the University and those acting pursuant to its authority deem appropriate.
    • Distribute the medium over the internet using formats that allow downloading and playback on mobile devices and personal computers, for the purpose of making the work available in any format through ITunes or other web-based service. 
    • Make and maintain more than one copy (hard-copy and/or digital copy) of the work for purposes of security, back-up and preservation. 

    As to any video, audio, photographic, digital, electronic, podcast, or any other medium, I understand and agree that the University shall have exclusive ownership of the copyright and other proprietary and property rights in the work. I release the University and those acting pursuant to its authority, from liability for violation of any personal, intellectual (including copyright) or proprietary rights I may have in connection with uses of the likeness or recordings authorized above. To the extent required, I hereby grant and assign all copyright in the podcast, video, audio, photographic, digital, electronic or any other medium utilized to the University. I waive any right to inspect or approve the final use(s) of the video, audio, photographic, digital, electronic, podcast or any other medium.

    I agree that the University shall have the right to remove the work from the media or web-based service at any time and for any reason.

    I release and hold harmless the University and its officers, agents, and employees from any claim or liability relating to the use of my name, likeness, identity, voice, photographic image, video graphic image and oral or recorded statements in the work, including all claims of compensation, damage for libel, slander, invasion of the right of privacy or any other claims based on, arising out of, or connected with the use of said recordings and podcasts.
