Day Training Policies and Participation Agreement

I certify that each time I leave my dog(s) with Grace Dog Training & Behavior, my dog is in good health and has not been ill with any communicable conditions in the last 30 days.*
At nap time during Gracie’s Day Training, all dogs will receive Gracie’s Freezies. These are made with all-natural, organic, dog-safe ingredients frozen inside durable natural rubber toys, including organic Greek yogurt (plain, no sweeteners), organic pumpkin puree, and organic peanut butter (salt-and sugar-free). If I do not want my dog to receive Gracie’s Freezies during rest time, I agree to provide either a rest-time chew appropriate for my dog’s size and chewing habits (no rawhides) or a stuffed toy or lick mat for enrichment and comfort.*
I understand and agree that my dog(s) must have current vaccinations for rabies (for dogs over 16 weeks of age), and distemper and parvovirus (for dogs over 8 weeks of age) before attending the center. I understand that my dog cannot attend the center without proof of these vaccinations.*
I understand and agree that my dog(s) must be spayed or neutered if they are older than 18 months before attending the center.*
I understand when leaving my dog in the care of Grace Dog Training & Behavior, I must provide a properly fitted harness, a 6 or 8-foot leash, chewies, and high value food rewards for treats in an amount appropriate for their stay. I understand if I do not provide these, Grace Dog Training & Behavior will purchase these items for me and an invoice in the amount of the total purchase price plus a 10% concierge fee will be charged to my credit card on file. *
I understand and agree that during my dog’s stay at Grace Dog Training & Behavior, s/he will be in direct contact with other dogs in the common area. I understand that in any group environment dogs may be exposed to communicable diseases, just like kids in school. This includes, but is not limited to illnesses like kennel cough. Clients are encouraged to discuss any questions or concerns they may have about this with a Grace Dog Training & Behavior team member. As hard as we work to prevent illness through vaccinations, thorough cleaning and sanitizing, we cannot guarantee that your dog will not become sick.*
The Grace Dog Training & Behavior staff and volunteers always work to ensure the safest environment possible for people and dogs in a boarding or training setting. I understand and agree that due to the inherent nature of dogs, my dog(s) may injure itself or be injured by another dog despite the reasonable and diligent care by Grace Dog Training & Behavior, and I hereby voluntarily assume the risk of any such damage or injury. I agree that Grace Dog Training & Behavior, its staff and volunteers, cannot be held liable for such issues.*
I understand and agree that I am solely responsible for the behavior and acts of my dog. I am financially responsible for any damage or injury caused by my dog(s) while my dog(s) is/are attending Grace Dog Training & Behavior’s Board & Train, Boarding, and/or Day-Training. I agree to indemnify Grace Dog Training & Behavior against any and all claims for injuries or illness arising out of the keeping of my dog(s) up to and including death.*
In the incredibly rare situation where my dog dies while in the diligent care of Grace Dog Training & Behavior, I understand and agree that the staff may transport my dog to my veterinarian or to a nearby veterinarian at the discretion of the staff at Grace Dog Training & Behavior. I do understand that I will be contacted first and foremost if such a situation arises.*
I understand and agree that in admitting my dog(s) to Board & Train, Boarding and/or Day-Training Grace Dog Training & Behavior, its staff and volunteers, have relied on my representation that my dog(s) is in good health and has not harmed or shown aggressive or threatening behavior towards any person or dog. Participation of my dog in training is at the discretion of the Grace Dog Training & Behavior staff and volunteers, who reserve the right to expel or temporarily remove dogs from training as they deem necessary to ensure the safety of all participants.*
I understand and agree that if any problem or medical emergency develops with my dog(s), the staff at Grace Dog Training & Behavior will act at their discretion to provide what they deem necessary for my dog’s well-being, including transporting my dog to my veterinarian or the nearest available veterinarian if an urgent situation arises. The staff will make every reasonable effort to contact me, my emergency contacts, and, when appropriate, my veterinarian before providing care beyond basic first aid. I acknowledge that my dog(s) will receive the necessary medical attention and that I remain fully financially responsible for any and all expenses incurred.*
I understand and consent that if my pet becomes sick or injured while at the training center, Grace Dog Training & Behavior staff may seek veterinary care as needed after contacting me and failing to reach me.*
I understand and agree that any and all video and photos taken while my dog is in the care of Grace Dog Training & Behavior can be used for teaching, marketing and any other purpose Grace Dog Training & Behavior sees fit and these will remain the sole property of Grace Dog Training & Behavior, though I may request copies of photos and video taken.*
I understand that if my dog is on any medication, the medication will be delivered in the original labeled prescription bottle. The dog should accept the medication easily in a treat, cheese, or peanut butter.*
I understand and agree that Grace Dog Training & Behavior staff may use products not specifically labeled for pets (such as Benadryl) if necessary for my pet’s care.*
I understand and agree that if my dog is taking medication, Grace Dog Training &Behavior must comply with state law. This law states, “Boarding/training facilities acting as agents for the owner, may administer prescription medication to pet animals being boarded as directed by the owner’s veterinarian. If the operators agree to administer prescription medications, the medications must be in the original container issued by a veterinarian or pharmacy and only be administered according to label directions. The label must include: client name/pet name, dosage, drug name, veterinarian’s name, and date issued."*
I understand and agree that all Day-Training, Boarding and Board & Train sessions are by reservation only and all fees must be paid prior to start date.*
I understand and agree that Grace Dog Training & Behavior has a 24-hour cancellation policy for Day-Training. If I cancel my reservation less than 24 hours in advance, I will forfeit one full day’s fees*
I understand and agree to drop my dog off at the training center for Day-Training no later than 8:00am. I understand that if I drop off after 8:00am, I will be charged a $25 late drop off fee. Grace Dog Training & Behavior starts training promptly at 8:00am and late dogs can be disruptive to the learning environment.*
I understand and agree to pick up my dog by 4:00pm. I understand that if I pick up after 4:00pm, I will be charged a $25 late pick up fee. Grace Dog Training & Behavior ends Day-Training promptly at 4:00pm to conduct private sessions and group classes and late pickup can be disruptive to the learning environment.*
I understand and agree to call Grace Dog Training & Behavior to arrange for pick up or drop off for my dog. Grace Dog Training & Behavior staff will always do our best to support and accommodate requests. I understand and agree to call Grace Dog Training & Behavior to communicate emergencies or delays in getting to the training center.*
No-show, non-cancel policy: I understand and agree that if I reserve a spot forDay-Training and do not show up nor cancel, I will forfeit that day’s fee. If I have bought an entire package and/or reserved a spot for my dog(s) for Day-Training, every day I do not show or do not cancel, I will be charged each of those reserved days that I do not show up.*
Day-Training, Boarding, or Board & Train: I understand and agree that my dog may spend up to four hours at a time in its crate for Boarding, or Board & Train (aside from overnight stays) to practice appropriate down time, rest breaks, and nap time.*
Boarding and Board & Train: I understand and agree that upon making a commitment to board or participate in a Board & Train program with my dog with Grace Dog Training & Behavior, 50% of the total due is required for a deposit immediately. I furthermore understand and agree that all boarding and Board & Train services must be paid in full 2-weeks prior to the program start date.*
Boarding and Board & Train: I understand and agree that if I cancel 2 weeks (or more) prior to boarding start date, I will forfeit a $50 administration fee only.*
Boarding and Board & Train: I understand and agree that if I cancel a reserved boarding stay within 2 weeks of my boarding start date, I forfeit the entirety of my boarding fees.Emergencies will be reviewed at the discretion of the staff at Grace Dog Training & Behavior on a case-by-case basis.*
