LIABILITY WAIVER AND RELEASE FORM
MOTIVATE & ELEVATE COACHING by Rebecca —2026 BREAKTHROUGH INTENSIVE- THE ELEVATION EXPERIENCE January 9, 2026. Location: DANCE DESIGNS DANCE COMPLEX - 265 Arnold Dr. Shepherdsville, KY 40165
1. Assumption of Risk
I acknowledge and understand that participation in dance, movement coaching, physical activity, and related workshops involves inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all such risks associated with participation in 2026 BREAKTHROUGH INTENSIVE- THE ELEVATION EXPERIENCE / DANCE DESIGNS DANCE COMPLEX - 265 Arnold Dr. Shepherdsville, KY 40165The Elevation Experience taking place January 9, 2026
2. Release of Liability
In consideration of being allowed to participate in this event, I, the undersigned, hereby release, waive, and discharge:
• Motivate & Elevate Coaching by Rebecca
• Rebecca Bass Maguet
• The 2026 BREAKTHROUGH INTENSIVE- THE ELEVATION EXPERIENCE & DANCE DESIGNS DANCE COMPLEX
• All staff, volunteers, agents, representatives, contractors, or affiliates of the above organizations
from any and all claims, liabilities, demands, damages, or causes of action, whether known or unknown, now or in the future, arising out of or in any way connected with my (or my child’s) participation in the event, including but not limited to those arising from negligence or carelessness.
3. Medical Treatment
In the event of a medical emergency, I authorize Motivate & Elevate Coaching staff or event personnel to seek necessary medical treatment for myself or my child. I agree to be financially responsible for any costs incurred as a result of such treatment.
4. Photo/Video Release
I grant permission for Motivate & Elevate Coaching by Rebecca and its affiliates to take and use photographs and/or video footage of me/my child during the event for promotional or educational purposes without compensation.
5. No Refund Policy
I understand and agree that all registration fees for 2026 BREAKTHROUGH INTENSIVE The Elevation Experience are non-refundable. This includes, but is not limited to, withdrawal due to scheduling conflicts, illness, travel issues, or personal circumstances. No credits or transfers will be issued.
6. Acknowledgment and Understanding
I certify that I am over 18 years of age or the parent/legal guardian of the minor named above. I have read this release of liability in full, understand its contents, and agree to its terms freely and voluntarily. I understand that this document is binding and that I am giving up certain legal rights by signing it.
