PARENTAL/GUARDIAN CONSENT TO PARTICIPATION

Please read this section carefully and then sign below to confirm your consent.  

I give permission for my child to take part in theASCENT process, which I understand has a duration of three years and will involve:

• three residential weekends per year
• weekly online 'PoDs' (online small groups)
• other meetings from time to time
• online small group mentoring, and one-to-one mentoring in person at the residential weekends
• other activities, as advised from time to time, including off-sire events and outreach activities

I confirm that the information given in this form is correct to the best of my knowledge.

I understand that certain standards of behaviour are expected and that participants can be removed from the process or from individual weekends and other events at the discretion of the organisers. I understand that in such cases it will be my responsibility to facilitate transport.

I understand that I must remain contactable throughout weekends and other events and that I must provide the organisers with all the relevant information they need to properly look after my child.

I give permission for theASCENT team to administer basic first aid and, if necessary, seek medical treatment for my child in the event of illness or injury. I understand that every effort will be made to contact me before such action is taken, but I authorise any necessary treatment in my absence.

I understand that theASCENT has a safeguarding policy and that relevant staff and volunteers are trained in safeguarding procedures. I give permission for contact details to be shared with safeguarding personnel if required for the welfare of my child.

PARENTAL/GUARDIAN CONSENT TO USE OF DATA

I give permission for theASCENT to collect and securely store personal information about my child as part of their participation in theASCENT process. I understand that this data will be used only for the purposes of administration, safeguarding, and communication relating to theASCENT, and will not be shared with third parties without further consent, unless required by law. Data will be stored in accordance with UK GDPR and theASCENT's privacy policy. I understand that I can request access to, or deletion of, my child’s data at any time by contacting theASCENT

I agree to take a full part in theASCENT, to abide by its rules, and to respect other participants. I understand that failure to do so may result in my removal from theASCENT with loss of monies already paid and potential liability for the rest of the year’s fees. I affirm that the information given in this form is correct to the best of my knowledge. I understand and agree to the terms and conditions noted on the 'Payment Information' and 'Parent/Guardian Information' pages. I understand that theASCENT will store the data on this form for the duration of the process and for the purpose of properly looking after me and administering the process. I understand that theASCENT will use my data to contact me regarding the arrangements of the process.

I CONFIRM that I have read the Safeguarding page on theAscent website and I consent to the mentoring process

I give theASCENT consent to store my home address, emergency phone numbers, and email address both during and after the process and to use that data to inform me of any courses, programmes, resources, events, or other opportunities, provided by theASCENT, CREW and others, which theASCENT believes may be useful or interesting to me.

I understand, and agree with, the fact that photo and video will be taken by theASCENT during weekends and other activities. I understand that such material will be stored by theASCENT and used for the purposes of future publicity. I understand that it is my responsibility to let those taking photos or video know if I do not wish to be included. I understand that theASCENT is not responsible for photos and video taken by third parties, including other participants
