I WILL FOLLOW THE AFTERCARE INSTRUCTIONS PROVIDED BY THE ESTHETICIAN.
I UNDERSTAND THAT INDIVIDUAL RESULTS MAY VARY, AND THE LONGEVITY OF TINTING MAY BE INFLUENCED BY VARIOUS
FACTORS.
I ACKNOWLEDGE THAT THERE MAY BE SOME DISCOMFORT DURING THE LAMINATION/TINTING PROCESS.

I RELEASE WAXED BY KRISSY, ITS EMPLOYEES, AND OWNER FROM ANY LIABILITY ASSOCIATED WITH THE EYEBROW LAMINATION, TINTING, AND WAXING SERVICES PERFORMED ON ME. I UNDERSTAND THAT UNFORESEEN REACTIONS OR
COMPLICATIONS MAY OCCUR, AND I ASSUME ANY RISKS ASSOCIATED WITH THE SERVICES.
I HAVE READ AND UNDERSTOOD THE INFORMATION PROVIDED ABOVE REGARDING THE EYEBROW LAMINATION, TINTING, AND WAXING
SERVICES. I HEREBY GIVE MY CONSENT TO DEZERA AT WAXED BY KRISSY TO PERFORM THESE SERVICES ON ME.

PICTURE RELEASE

I authorize Waxed by Krissy, its representatives, employees, and contractors to take photographs and/or videos of me or parts of my body during or after my service.

I understand and agree that:

- These images may be used for marketing, advertising, educational content, or promotional materials in digital or print form (including but not limited to Instagram, Facebook, Tik Tok, website, and training purposes).
- I will not receive compensation for the use of these images.
- My name and personal information will not be disclosed unless I give specific written permission.
- I can choose whether or not my face or identifying features (e.g., tattoos, scars) are shown.
- I may revoke this consent in writing at any time, but understand that materials already published cannot be retracted.
