Dear Sir/Madam,

This is to inform you that I am cancelling my subscription/services with Freedom Repair Specialists

As per the contract, once this form is filled out it will take 30 days to fully complete the cancellation process in which Weekly Charges will continue as per contract with you. Within this time your file is still open and all our services are still open to you. All clients and applicants who wish to cancel must fill out this form. Any client who does not fill out this form WILL NOT BE CANCELLED.
